The ability to share patient-client data between providers and programs is essential to providing comprehensive services, community supports, and care across the continuum, including tackling the complex challenges that come with addressing social drivers of health. To enable this data sharing, we need mechanisms to gather, track, and manage individual’s data-sharing consent, and robust interoperability to ensure data has context, so that it is reliable, actionable, and understood.
