Sudden inspiration! :P And this is the result :) I've been wanting to do this for a while, and i've had this idea stuck in my head. Its such a beautiful song :')
I think the plotline is pretty straighforward, but here is the basic outline:

Zuko and Katara are and have been otgether for a while now, but then, unexpectantly, Aang kisses Katara, having feelings for her for a while and being unable to contain them. Althought Katara doesn't love Aang back in that way, and the kiss meant nothing to her, she still decides to tell Zuko, wanting trust and no hidden secrets between them.
When he finds out, his reaction is not good. He is at first frozen, then sad and hurt. Katara explains it meant nothing, and although Zuko tries to believe her, he can't. Even a while after the incident, he keeps imagining them kissing, again and again, and this drives him away from Katara. He starts mysteriously leaving, wanting to get away from her, because everytime he looks at her, he is reminded of the image of her and Aang. As their fights get worse and worse, he stars changing into a scary person that Katara isn't familiar with, and it's starting to scare her.
Eventually, he becomes really aggressive, breaking furniture and shouting. And he constantly has nightmare's of himself growing old, mad and alone, until he cant' take it anymore. After waking, he writes Katara a note, and leaves it for her to find before leaving. However, Katara discovers the note earlier than he had anticipated. Realising it's a suicide note, she sprints out of the house, towards where he said he would be.
Meanwhile, Zuko is stading on top of a cliff, rain pouring on him. He looks out at the landscape around him, preparing to say his final goodbye's to life. Katara is running closer, and shouts for him. Zuko's eyes flicker to her, linger on her for a second, thne turn away, and a tear trickling down his face. Then, he gets struck my lightning.
As he falls, his eyes widen with shock. He realises he's made a mistake, as his dillusions that clouded his judgement finally sift away. Katara screams his name, but it is too late. She runs towards his body on the ground, and tries her best to heal him. When she realises it isn't going to work, she still doesn't lose hope. However, Zuko stops her, and says something in a weak voice. Katara replies.***** But then, the life filters away from his eyes, leaving an empty shell, leaving Katara alone, to weep for her loss.

****** This is something along the lines that they might have said:
Zuko- "Katara, it's too late now. I'm sorry for this... what I've put you through..."
Katara- "Dont' be stupid. You're gonna be fine. You can't die... I love you....
*zuko dies*
