Here is an example of a failing egr cooler and stuck open egr valve. This is an indication of the egr cooler starting to fail or in rare cases at least out here in the So Cal desert of too much idle time depending on calibration. When I see this it's almost always an egr cooler and oil cooler now or within a few months.
