With unprecedented access to the United States Coast Guard’s Marine Board Investigation, and interviews with some of those most affected by the tragedy, this gripping documentary unpacks a series of warnings, and a chain of failure.
With speculation rife on social media around the world about the cause, the U.S. Coast Guard launched its investigation to determine what had gone wrong. The public hearing gathered testimony from key witnesses and experts, raised questions about oversight and accountability, and brought critical information about Titan’s design, testing, and operations into the public record.
Through access to the hearings and testimony, the film explores events that led to the disaster — examining decisions regarding vessel design, maintenance, and safety. Through interviews with those closest to the events, including industry experts, key figures from the US Coast Guard investigation team and one family member of two of the victims, the documentary explains definitively what caused the implosion and offers an in-depth look at the systemic issues that, over the preceding years, contributed to the tragedy. Understanding what failed — and why — may help prevent future disasters in deep-sea exploration.
