Two planes carrying 349 people speed towards each other in the evening skies above Delhi.

All of these systems and infrastructures make up the foundations for the pyramid of aviation safety, on top of which pilots, and air traffic controllers sit. What you’re about to see, is the shocking story of what happens when the foundations of this pyramid, are rotten.

In the dead of the night, halfway over the Atlantic ocean, an Airbus A330 carrying 306 passengers and crew suddenly runs out of fuel. The lights go out, the oxygen masks drop, and the noise of the engines is replaced with an eerie silence. In pitch darkness, the giant aircraft begins drifting down towards the ocean below. On the morning of November 4th, 2010, 440 passengers and 29 crew boarded Qantas flight 32, at Singapore’s Changi airport. The aircraft they were flying on, a state-of-the-art Airbus A380, would go on to suffer a catastrophic uncontained engine failure shortly after takeoff. Almost 500 lives hung in the balance as the five pilots in the cockpit did everything they could to save the crippled aircraft. This incident is the most serious to happen to an A380 to date, and the crew’s handling of it has been highlighted by aviation experts as being one of the finest examples of airmanship in the history of aviation.

In the early morning of December 28th, 2014, 156 passengers boarded an Airbus A320 operating as Indonesia Air Asia flight 8501, at Juanda International airport in Surabaya, Indonesia. They were bound for Singapore’s Changi airport, a two and half hour journey. But a perfect storm of human fallibility, deficiencies in pilot training, and mechanical error would come together to ensure that the flight never made it to its destination.

On December 1st, 1981, a brand new McDonnell Douglas MD-82 carrying 173 tourists made its approach to Ajaccio airport on the French island of Corsica.
This was the pilots’ first time flying into this airport, and they were unfamiliar with the area. As the aircraft descended into the clouds above the island, a series of misunderstandings began to take place between the pilots and air traffic control.
Minute by minute, as the aircraft descended, these misunderstandings were compounded, and the shared mental pictures between the pilots and air traffic control began to diverge, with disastrous consequences.
Flight 1308 sent shockwaves across the aviation industry, and raised serious questions about everything from the way pilots and air traffic controllers communicate, to the way that approach charts are designed. The story Inex-Adria flight 1308 shows that under certain conditions, a tiny misunderstanding can snowball into a full-blown catastrophe.

On November 12th, 1995, an American Airlines MD-83 carrying 78 passengers and crew is making its final approach to Bradley International airport in Connecticut. It’s running late, and the crew are eager to make it to the airport. The area is blanketed by torrential rain and turbulence, and the two pilots are struggling to keep the aircraft on course. The winds are so strong that the control tower had to be evacuated, and is now occupied by a lone supervisor who hasn’t manned a tower in years.
