We’ve spent decades fixating on fat, blood sugar, and cholesterol when the real story is muscle.
Skeletal muscle is the largest endocrine organ in your body. It’s where glucose is stored, where insulin sensitivity begins, and where your metabolic health is decided.
Yet, 80% of Americans don’t train, and they lose muscle every decade after 30, even faster if they’re sedentary.
When muscle breaks down, everything else follows. But when you build it, you build resilience against aging, chronic disease, and decline.
The future of medicine isn’t just about managing disease.
It’s about building healthy muscle. Because strong muscle is the strongest form of prevention.

But it’s much more than that. It’s the foundation for nearly every system in our body.
Here are some facts…
As we age, our muscles become less sensitive to the signals from food. This means we need MORE protein, not less, to keep our metabolism optimized and maintain strength.
Research shows that older adults often need up to 1 gram per pound of body weight just to keep muscle tissue responsive.
And it’s not just about quantity, it’s about quality.
To actually “turn on” muscle growth, we need enough of the essential amino acid leucine (around 2.5 grams per meal).
That’s why animal proteins like eggs, beef, and whey are so effective, they give your body the raw materials it needs to build, repair, and thrive without excessive calories.
When you meet the needs of your muscle, you’re entire body transforms, inside and out.
Because muscle health is metabolic health.
Most people are overweight because they eat too much, plain and simple.
It’s not a sexy answer, but it’s the truth.
And unfortunately, most people are terrible at knowing just how much they are eating.
For example, one study found that men and women with obesity underestimated energy intake by an average of 20%.
Even worse, a group that claimed to have a broken metabolism and not be losing weight while eating under 1,200 calories per day were underestimating by an average of 1,050 calories, or 47% of actual intake.
If you are struggling to lose weight and you don’t track what you eat, Occam’s razor suggests that you are likely eating more than you think.

Here’s the framework:
Protein comes first. It’s structure. It builds and repairs muscle, supports immunity, and drives metabolic health. Think of it as your non-negotiable.
Carbohydrates are conditional. They’re the riskiest fuel, spiking insulin and blood sugar if mismatched to your activity level. Use them strategically around training, when your muscles can actually use them.
Fat is the fallback. Once protein and carbs are accounted for, fat fills in the rest. It’s the safest energy reserve and supports hormone production, brain health, and satiety.
Choosing between carbs and fats is not a moral choice. They’re just fuels. Protein is the priority. The rest depends on how much you move.
Aging comes in catabolic crises. Illness, injury, hospitalization, even a week in bed… these are the moments when muscle vanishes fastest.
Older adults can lose 2–5 pounds of muscle in just 10 days of bed rest, and most never fully recover.
That’s why building and maintaining muscle isn’t about vanity, it’s about preparation.
Muscle is your reserve tank for life’s inevitable setbacks. The stronger you are going in, the better you bounce back.

But the truth is more complex. Your body’s ability to manage carbs is limited, and that ability depends on muscle.
At rest, only a small fraction of carbs are used by your brain and organs, and even less by sedentary muscle. When your muscles are inactive and glycogen stores are already full, there’s nowhere left for those carbs to go.
This is why muscle health is central to metabolic health. Trained muscle doesn’t just move you—it acts as a sink for glucose, helping balance your entire fuel system. Without it, carbs become a liability. With it, you can actually “earn” your carbs.
So the question isn’t whether carbs are good or bad. It’s whether your muscle is healthy enough to handle them.
Since the 1970s, U.S. diets shifted heavily toward chicken and ultra-processed foods, and as a result, the micronutrient density of our protein choices has dropped.
Key nutrients like iron, zinc, selenium, riboflavin, B6, and B12—all abundant in red meat—have declined in the American diet.
Swapping steak for chicken nuggets is a downgrade in nutrient quality. If you want to support muscle, metabolism, and long-term health, prioritize whole-food protein sources that deliver the full package of amino acids and essential micronutrients.

Clinical trials show that 25–40% of the weight lost on GLP-1s comes from lean mass, including muscle.
Muscle is your biggest site for glucose disposal, a key regulator of metabolism, and a major factor in long-term weight maintenance.
History backs this up: analysis of the Minnesota Starvation Experiment showed that the best predictor of rebound weight gain wasn’t how much fat people lost, it was how much muscle they lost. Strip away too much muscle, and your body fights harder to put weight back on.
The fix? Two non-negotiables:
Prioritize protein: aim for ~1.6 g/kg of target body weight daily, eating it first at meals.
Lift weights: 2–4 times per week to give your muscles the signal to stay strong.

It isn’t. It’s common, but it’s not inevitable.
The truth is, muscle is a choice. You can build and protect it, or you can ignore it and let it waste away.
Cross-sections of muscle tissue tell the story: a healthy 70-year-old who trains looks nearly identical to a 40-year-old. Meanwhile, a sedentary 74-year-old’s muscle looks marbled with fat, weak, and dysfunctional, almost like a ribeye steak.
That difference isn’t age. It’s activity.
Hormones decline as we get older, yes. But the bigger problem is that muscle becomes less sensitive to dietary protein and exercise stimulus. That means the habits that worked in your 20s won’t cut it in your 50s and beyond. You need more protein. You need to train smarter.
Weakness is not your fate. Muscle is your way out.

Because muscle isn’t just for strength or looks. It’s your metabolic sink, your armor against disease, and the foundation that connects physical capacity with mental resilience.

The catch? Muscle can’t be bought. It can’t be bargained for. It has to be earned and maintained.

Do you think your organs stop functioning while you sleep? Of course not. They require a steady supply of amino acids to keep you alive 24-7.
About 85% of the amino acids your body requires after an overnight fast come from muscle tissue, and rates of muscle protein breakdown are about 30% greater than rates of muscle protein synthesis.
That’s a net loss of muscle tissue.
That’s why your first meal is so powerful. It’s not about the time you eat (8 a.m. or 11 a.m. makes little difference), it’s about what you eat. A donut or sugary cereal sends one signal: more hunger, more blood sugar swings, and no support for your muscle.
But 30–50 grams of high-quality protein? That flips the switch. You supply the essential amino acids needed to rebuild muscle, reduce cravings, and set your metabolism on the right track.

There are so many ways to set up this mass building game plan, so find a way that works best for you.
This is what I’ve found successful for myself and many others:
— Set your calories at 10-20% above maintenance requirements.
— Eat 1.0 to 1.2 grams of protein per pound of body weight
— Eat 1.4 to 3.6 grams of carbs per pound of body weight
— Eat 0.3 to 1.0 grams of fat per pound of body weight
— Eat four meals per day with at least 40 grams of protein per meal
If you prefer eating more carbs, go lower far.
If you prefer eating more fat, go lower carb.
But always hit your protein and lift heavy.
When we measure “metabolic syndrome” : high blood sugar, blood pressure, lipids, and high waist circumference, excess body fat, we’re really measuring unhealthy muscle. The problem isn’t just the obesity, it starts in skeletal muscle, often decades before disease shows up.
Stop chasing symptoms. Focus on building and protecting muscle—the true key to survivability, longevity, and resilience.

The modern diet is built on low-protein, high-sugar, ultra-processed foods, and it shows.
With rates of overweight and obesity at over 75%, we’re eating enough to be overfed, but not enough of the right things to be well-nourished.
Every trip to the grocery store shapes your strength, energy, and resilience. A diet with a high intake of UPFs was associated with a 28% higher risk of mortality in a diverse multinational study (PURE study).

Erections: A “Check Engine Light” for Your Health
One of the most striking points from the discussion was Dr. Kohler’s perspective on erectile dysfunction (ED). He described it not just as a sexual problem, but as one of the earliest "check engine lights" for heart disease. The blood vessels to the penis are smaller than those in the heart, so plaque buildup (atherosclerosis) affects them first. Persistent erection issues can signal vascular problems that may soon affect your heart. As Dr. Kohler put it, "If the penis is failing, assume you'll have heart problems soon until proven otherwise." The good news: what’s good for your heart—exercise and a healthy diet—is also good for your penis.
The "Use-It-or-Lose-It" Organ
The conversation also tackled the surprising phenomenon of penile shortening. When the penis goes into a "garage" for an extended period—meaning there is a consistent lack of erections—it can lose length. Dr. Kohler shared that a consistent lack of erections for just three months can lead to a loss of 1-2 centimeters in length due to the formation of scar tissue. This is why he calls the penis a "use-it-or-lose-it" organ. The key to maintaining penile health is to exercise the penis through erections, and tools like a vacuum erection device can be used for penile rehabilitation.
Muscle Mass, Testosterone, and Sexual Function
For many men, this is where the conversation gets personal. Dr. Kohler highlighted the direct correlation between muscle mass and erectile quality. Studies show that men with more muscle mass have better erections and higher testosterone levels. The reverse is also true: men with less muscle mass often have lower testosterone, lower libido, and worse erections.
In fact, a modest 10% decrease in body weight can be as effective as ED medication in improving erections. Regular exercise also plays a huge role. Dr. Kohler cited a study that found 150 minutes of exercise per week decreased the rate of ED by 20%, while 300 minutes decreased it by nearly 40%. The takeaway is clear: if you want to improve your sexual function and overall longevity, you have to move iron and get strong.

Before considering surgery like a penile implant, Dr. Kohler urges men to focus on diet, exercise, sleep, and stress control—the first line of defense against ED and chronic disease.
