Heller Myotomy with Dor Fundoplication Animation for the Treatment of Achalasia Fauquier ENT 2021-02-19 | In patients suffering from achalasia, the LES (lower esophageal sphincter) remains abnormally closed preventing food and liquids from easily passing into the stomach, especially given weak or even absent esophageal peristalsis. Food is essentially falling by gravity after swallowing. Given the delay in food passage into the stomach, the lower esophagus dilates. Symptoms stem from food failing to clear the LES leading to food regurgitation, difficulty swallowing, and even chest pain. Interestingly, achalasia symptoms are often confused with reflux.Should conservative medical management fail to resolve symptoms, a Heller myotomy can be performed to widely open the LES by incising it in order to make it easier for food to fall into the stomach. Esophageal peristalsis is not able to be restored but resistance to food passage can at least be reduced surgically. Because the LES is wide open afterwards, the patient is at risk for significant reflux. Bleach Baths for Recurrent Skin Infections Fauquier ENT 2022-10-31 | For individuals who suffer from severe eczema, folliculitis, impetigo, hidradenitis, boils, and/or skin infections... bleach baths may be very helpful to help resolve these skin conditions. Keep in mind that bleach baths is no different than chlorinated water in a swimming pool. Bleach is simply chlorine! At the strengths that are recommended, a bleach bath is simply like a double strength chlorine swimming pool. Just like a swimming pool, it is not a problem if small amounts are ingested or get into eyes. And just like how chlorine in a swimming pool can decrease the overall bacterial load in the pool water, bleach baths can similarly decrease the overall bacterial load on the skin that keeps getting infected. Steps are as follows:1. Start by adding lukewarm water to fill a tub for a normal bath (about 40 gallons).2. Put 1/4 cup of common liquid bleach (for example, Clorox) into the bath water if tub is 1/2 full. It would be double that if the tub is full. Check the bleach bottle to make sure that the concentration of bleach (also known as sodium hypochlorite) is about 6 percent.Clorox from Amazon: amzn.to/3fkeviP3. Completely mix the added bleach in the water. This should create a solution of diluted bleach (about 0.005 percent), which is just a little stronger than chlorinated swimming pool water.4. Soak in the chlorinated water for about 10 minutes.5. Thoroughly rinse the skin clear with lukewarm, fresh water at the end of the bleach bath.6. As soon as you’re finished rinsing off, pat dry. Do not rub dry, as this is the same as scratching!7. Immediately apply any prescribed medication and/or emollients.8. Repeat bleach baths two to three times a week or as prescribed by the physician.Do NOT use undiluted bleach directly on the skin. Cellulitis is when the infection spreads into the deeper layers of the skin. Video specifically focused on a cluster of lesions found on the back of the right thigh. It was treated with mupirocin and cephalexin, but the infection continued to get worse. Given the lack of improvement, cultures were obtained from the oozing sores which revealed a multi-drug resistant staph infection. By the time this information was finally obtained, cellulitis was present given the surrounding redness and induration. Appropriate culture-directed antibiotics was immediately started along with steroids to help with inflammation. Immediate improvement was seen within 24 hours. As the infection resolved, the affected skin takes on a more bruised appearance. After oral antibiotics were completed, topical skin care alone was pursued until the skin completely healed. Ultimately, it took nearly a month for the skin to return back to normal. Hopefully this 30 day time-lapse showing how the skin looks from from initial onset, to worsening, to finally resolution will help patients (and parents) understand visually the overall healing time course.There is no single "right" way to treat such skin infections. Selective Cornea Transplant - Endothelial Keratoplasty (DMEK, DSEK) Fauquier ENT 2022-08-11 | The cornea is the transparent, dome-shaped surface of the eye where light enters. It is a large part of the eye's ability to see clearly. Unfortunately for some patients, the inner lining of the cornea may become cloudy leading to vision loss, most often from Fuch’s Dystrophy or Bullous Keratopathy. Treatment is often pursued before the cornea becomes completely clouded. As such, treatment to restore vision requires a highly selective cornea transplant involving removal and replacement of just the diseased inner lining of the cornea with a donor graft, either DSEK (Descemet’s Stripping Endothelial Keratoplasty) or DMEK (Descemet’s Membrane Endothelial Keratoplasty).
