When facial fat is lost quickly, we can see hollowing in the cheeks, temples, and under the eyes, along with loose skin that can create a tired or aged appearance. This can be especially noticeable in older patients who are already experiencing natural facial volume and collagen loss.
Treatment depends on what the face needs. Dermal fillers or fat grafting can help restore lost facial volume, while a Deep Plane Facelift may be appropriate when there is significant skin and deeper tissue laxity. In some patients, combining a Deep Plane Face and Neck Lift with fat grafting can address both sagging and volume loss for a more natural-looking facial rejuvenation.
The key is treating the anatomy, not simply the weight loss.
Pinterest: pinterest.com/ocsurgeonWhats the Best Age for a Facelift?Dr. Kevin Sadati2026-09-20 | What's the Best Age for a Facelift? Justine's Family Proves It Isn't About a Number
For people with hereditary fullness in their neck, age isn't necessarily the right question. A 60 year old can have neck fullness just as much as a slender 30 year old. Muscles and deep structures of the neck and genetics play a bigger role than weight or age. More important than when someone gets a facelift is whether their anatomy is right for it. That will vary from person to person.
Justine at age 32, found this out the hard way. Her first procedure, liposuction and chin augmentation, removed superficial fat under the skin but couldn't treat the deep fat layer and muscle laxity. She ended up disappointment and barely noticeable results. She saw her younger sister who decided against liposuction, consulted with Dr. Sadati, had the correct treatment a deep plane neck lift and loved the results. Then, their mother later had a preservation deep plane face & neck lift and is over the moon with the results.
Justine then consulted with Dr. Sadati, who recommended a mini deep plane facelift, deep structural neck lift, buccal fat repositioning and fat grafting. Now two months later she is extremely happy with her decision. Liposuction alone thins the fat under the skin but doesn't affect the deep fat and muscle underneath, where her neck fullness lay.
Now her face and neck look lifted but not the pulled look. Right procedure, not right age, makes all the difference.

At 46 years young, Deanna felt this was the right time for her. It was a personal choice. She feels good, and she wanted her reflection to match the way she feels inside.
When considering a Deep Plane Facelift, choosing the right facelift surgeon is essential, someone who understands your goals and prioritizes natural looking facelift results without changing what makes you, you.
Every facial rejuvenation journey is personal, and age alone doesn’t tell the whole story.

Kim’s 68th year wasn’t just about looking younger. More importantly, she wanted to feel more confident and vital inside.I performed a Preservation Deep Plane Face and Neck Lift, buccal fat repositioning with fat transfer and CO2 laser resurfacing. All procedures were performed under twilight sedation without general anesthesia.
In a Preservation Facelift, the deep tissue instead of skin is lifted, by releasing the retaining ligaments. By vertically repositioning the deep facial layer, I avoid the overly pulled look common in traditional facelifts. Just two months after her surgery, Kim says she felt more like herself.
“I have more confidence, more energy, and I’m really looking forward to the rest of my life.” Kim
It’s not my goal to change who you are. Rather, I want to help you look more like a natural, refreshed version of yourself.

#Facelift #PreservationFacelift #DrSadati #PlasticSurgery #deepplanefaceliftHow long does a facelift last? #preservationfaceliftDr. Kevin Sadati2026-09-15 | 8 months post-op, and her results continue to reflect exactly what we set out to achieve: natural, balanced facial rejuvenation.
At 62 years young, she wanted her reflection to better match the vibrant way she feels inside. I created a personalized surgical plan that included a Deep Plane Face and Neck Lift, Temporal Brow Lift, Lower Blepharoplasty, TCA Peel under the eyes, and Fat Grafting to restore youthful balance and harmony without changing what makes her look like herself.
Now, 8 months after surgery, her face and neck look refreshed, refined, and beautifully natural. This is why every facelift I perform is tailored to the individualnever a one size fits all approach.
#Facelift #PreservationFacelift #DrSadati #PlasticSurgery #deepplanefaceliftWhen Will I Look Normal After a Facelift?Dr. Kevin Sadati2026-09-13 | At 65 years young and soon to be 66, Debbie had one simple goal with her facial rejuvenation: to look in the mirror and feel good about what she sees.✨
During the early days of her Deep Plane Face and Neck Lift recovery, she experienced swelling especially around days three and four. Even when things looked a little different, she reminded herself to trust the process.
Swelling is a normal part of facelift recovery, and the early appearance is not the final result. As the swelling gradually subsides and the tissues continue to heal and settle, her natural looking facelift results will continue to reveal themselves.
I love Debbie’s perspective because a Deep Plane Facelift at 65 isn’t about changing who you are. It’s about restoring natural facial harmony and helping the person you see in the mirror better reflect how you feel inside.

#Facelift #PreservationFacelift #DrSadati #PlasticSurgeryHow Does a Deep Plane Facelift Work?Dr. Kevin Sadati2026-09-11 | This beautiful 61 year-young woman came to see me after having a previous facelift and upper eyelid surgery, hoping to refresh her appearance once again and address new changes in her midface, lower face, and neck, as well as lower eyelid puffiness and skin texture.
Her goal was simple and thoughtful: to look like a younger, healthier version of herself without looking overdone or surgical.
To achieve this, I performed a preservation deep plane face and neck lift, temporal brow lift, lower eyelid surgery, and full-face laser resurfacing. In the early healing phase, the laser resurfacing caused redness, which can be unsettling for some patients. She was incredibly patient and trusted the process of recovery, knowing that true results take time to reveal themselves.
Six months later, her transformation is truly stunning. There is a dramatic improvement in facial balance, skin texture, and overall youthfulness, yet there are no visible signs of surgery. This is what we call an undetectable facelift. Her face looks refreshed and natural, not tight or artificial.
What makes her result so special is that people see her as well-rested and radiant, not “operated on.” This is the essence of modern facial rejuvenation. Real transformation should be noticeable, but surgery should never be detectable.
I am grateful she trusted me with her face and honored to be part of her journey toward renewed confidence and beauty.
Pinterest: pinterest.com/ocsurgeonWhen Is It Time for a Second Facelift?Dr. Kevin Sadati2026-09-11 | At 65 years young, she came back to see me six years after her previous facelift, ready for a more comprehensive facial rejuvenation to restore natural harmony and address the changes that come with time.
For Debbi, I performed a deep plane face and neck lift, combined with an endoscopic brow lift, submandibular gland reduction, fat grafting, buccal fat reduction, muscle tightening, and a lip lift.
Each procedure was carefully selected to create balanced, natural looking results rather than changing who she is.
All procedures were performed under twilight sedation without the need for general anesthesia.
Thoughtful planning is the foundation of exceptional plastic surgery, and I can’t wait to share her transformation as she heals.
Pinterest: pinterest.com/ocsurgeonWhat’s the difference between a traditional facelift and a Preservation Deep Plane Facelift?Dr. Kevin Sadati2026-09-09 | What’s the difference between a traditional facelift and a Preservation Deep Plane Facelift?
A traditional facelift typically focuses on tightening the SMAS and repositioning the skin. With my Preservation Deep Plane Face and Neck Lift, I release the deeper retaining ligaments and reposition the facial tissues in a more vertical direction, with less reliance on tension placed on the skin.
My goal with facial rejuvenation is never to create a tight or pulled appearance. It’s to restore natural facial harmony while creating natural looking facelift results designed to age beautifully over time.
Every face is different, and the right facelift technique should always be tailored to your anatomy, aging pattern, and goals.
#Facelift #PreservationFacelift #DrSadati #PlasticSurgery #deepplanefaceliftIs 65 years old too late for a facelift? #preservationfaceliftDr. Kevin Sadati2026-09-06 | At 65 years young, Dawn came back to see me for a touch up after a previous facelift. Her goal wasn’t to look different, but to refine her results and have her appearance better reflect how she feels inside.
I performed a Deep Plane Facelift and Neck Lift, combined with CO₂ laser resurfacing of the face, neck, and chest to improve her skin tone, and texture.
Recovery takes patience, and Dawn experienced swelling along the way, but she trusted the process and gave her results time to mature.
Now at 6 months post-op, she’s enjoying a more defined chin and jawline, refreshed skin, and most importantly, a result that still looks naturally like her.
#Facelift #PreservationFacelift #DrSadati #PlasticSurgery #deepplanefaceliftCan a Previous Facelift Be Corrected? #preservationfaceliftDr. Kevin Sadati2026-09-03 | Today’s patient is 67 years young and came to me after having a SMAS facelift and coronal brow lift about four years ago. She was still experiencing laxity through the face and neck, prominent submandibular glands, and imbalance in the brow and upper eyelids.
For her revision, I’m performing a Preservation Deep Plane Face and Neck Lift with submandibular gland reduction and platysmaplasty, along with an endoscopic temporal brow lift, upper eyelid revision, fat grafting, and laser resurfacing. Revision surgery requires a very thoughtful approach.
My goal is to restore balance and reposition the deeper tissues while creating a natural result that feels harmonious with her face.

We work hard, but there’s always room for a good laugh along the way.Can Buccal Fat Be Repositioned Instead of Removed?Dr. Kevin Sadati2026-08-28 | Not every patient needs buccal fat removal.
For my 49-year-old patient, preserving her natural facial volume was an important part of the surgical plan. Rather than removing the buccal fat, I kept it and repositioned it to help restore balance and support where she needed it most.
Her personalized facial rejuvenation included a Preservation Deep Plane Face and Neck Lift, gland reduction, buccal fat repositioning, facial fat grafting, and a corner lip lift.
The goal is never to simply remove fat or tighten skin. It’s about understanding what should be reduced, what should be preserved, and what can be repositioned to create a natural, harmonious result.
#Facelift #PreservationFacelift #DrSadati #PlasticSurgery #deepplanefaceliftRevision vs. Redo Facelift: What’s the Difference?Dr. Kevin Sadati2026-08-25 | Not every second facelift is the same.
Revisional facial surgery is about addressing concerns or complications from a previous procedure. I typically recommend waiting about a year to allow the tissues to fully heal before considering revision surgery.
A redo facelift is different. This is often for patients who had a facelift 10-15 years ago and have naturally aged over time. In these cases, I may perform a Preservation Deep Plane Facelift, sometimes combined with fat grafting, a brow lift, or eyelid surgery for comprehensive facial rejuvenation.
Every patient is different, and the right approach always starts with understanding their previous surgery, anatomy, and goals.

#Facelift #PreservationFacelift #DrSadati #PlasticSurgeryDeep Plane Facelift Recovery: Week by WeekDr. Kevin Sadati2026-07-31 | One of the most common reactions I see after a deep plane facelift is concern about the swelling. It’s completely understandable, but it’s also completely normal.✨
Facial rejuvenation is a process. Whether it includes a deep plane face and neck lift, eyelid surgery, a brow lift, fat grafting, laser resurfacing, or a lip lift, your face needs time to heal. During the first week, swelling and redness are expected.
The most important part of recovery is patience. Give your body the time it needs, and you’ll begin to see the transformation unfold week by week. Just as important is maintaining a positive mindset.
Healing is a journey, and understanding what to expect can make the experience much less stressful.
The beautiful, natural looking results you’re hoping for don’t happen overnight, they continue to improve over the months.
Trust the process, stay positive, and remember that every day brings you one step closer to your final result.

#Facelift #PreservationFacelift #DrSadati #PlasticSurgeryNumbness & Tingling after a facelift.Dr. Kevin Sadati2026-07-23 | One of the most common questions I receive after a deep plane facelift is about numbness during recovery. Facelift healing is a gradual process, and understanding what to expect can make recovery much less stressful.
The numbness you feel after a facelift is completely normal and something I discuss with every patient before surgery. As the tiny sensory nerves heal, it's common to experience numbness, tingling, or even brief "electric shock" sensations during the first few months.
These nerves typically regenerate over the course of 3–6 months, and sensation gradually returns.
Healing is a process, and these temporary changes are a reassuring sign that your body is recovering just as expected.

I do say it often, because I truly believe it. Every patient has something beautiful about them, and after surgery it’s incredibly rewarding to see that confidence shine through even more.
My goal has never been to make someone look like a different person. It’s to restore what time has changed and help each patient look refreshed, natural, and like the most youthful version of themselves. Every face is unique, every treatment plan is customized, and every result tells its own story.

Three days post-op, and I know this photo looks intense. That redness isn’t bruising, it’s her skin actively resurfacing from a TCA chemical peel, part of a much bigger story.
This patient’s face tells a story most people never see. A snowboarding accident shattered her jaw, leading to 18+ reconstructive surgeries just to rebuild function. Years of fillers tried to lift jowls that gravity and improve old trauma. Sun damage aged her skin living in Southern California.
So I didn’t do “a facelift.” I did a facial rejuvenation: a Preservation Deep Plane Face and Neck Lift, fat grafting to restore lost volume, upper eyelid surgery to open her eyes back up, a chin implant to finally bring harmony to a jawline rebuilt by trauma, and a TCA peel to refresh the surface, bringing harmony to her face.
Healing takes time. Redness fades. Results don’t happen on day three, they happen over months.
#Facelift #PreservationFacelift #DrSadati #PlasticSurgeryCan a young person get Deep Plane Neck Lift? #necklift #buccalfatremoval ##doublechinDr. Kevin Sadati2026-07-10 | She is 26, at an ideal weight, healthy and radiant, yet for years since adolescence, she had lived with a double chin, prominent cheek fat and a dorsal nasal hump that never reflected her inner feelings. None of it was related to aging; it was simply a reflection of her genetic make up. This represents a quiet struggle that is not discussed enough.
Young patients who feel older than their chronological age, not due to time, but due to genetic factors are excellent candidates for facial contouring and rhinoplasty.
My proposed procedures to improve her facial features: a deep plane neck lift, reduction of cheek fat, fat grafting to the pre-jowl area, and rhinoplasty to achieve an overall sense of facial harmony.
The result is a beautiful face in harmony with its true character. I am deeply grateful to her for sharing her experience with all of us.✨

At 65 years young, she came back to see me six years after her previous facelift, ready for a more comprehensive facial rejuvenation to restore natural harmony and address the changes that come with time.

Happy Independence Day! ❤️🤍💙 Dr. Kevin Sadati and the entire team at Gallery of Cosmetic SurgeryWhat is different about a facelift for men compared with women? #faceliftmen #malefacelift #neckliftDr. Kevin Sadati2026-07-02 | Meet Jeff a 59 year old nice gentleman , and one of the most complex cases I have ever operated on.
Jeff had severe sleep apnea due to a hereditary heavy neck. Two INSPIRE devices pacemaker like implants implanted in his neck that stimulate the nerve controlling tongue movement to keep the airway open had both failed because the fullness of his neck created too much anatomical pressure air pipes.
When he came to me, the previous device wiring was still in his neck, wrapped around the hypoglossal nerve and a bunch of scar tissue. Any damage would have affected his ability to speak and swallow. With careful specialized maneuvers, I performed a preservation deep plane neck lift removing what Jeff called his “turkey neck” while also relieving the pressure behind his sleep apnea.
Today, Jeff sleeps better. He speaks clearly. No nerve damage. And he finally has the neck he always should have had.
Bravo Jeff, for trusting me and for sharing your story so others know there is always a path forward.

This lovely 70 year old patient came to see me for a refresh more than a decade after her SMAS facelift and upper eyelid surgery, wanting to look her best.
For her rejuvenation, I performed a deep plane face and neck lift using my preservation technique to naturally elevate the neck, jawline, and mid face. I also completed laser resurfacing to improve skin quality and restore a healthy glow.
The transformation is subtle yet powerful, beautiful facial rejuvenation that leaves her looking refreshed, natural, and decades younger. That's the artistry of modern plastic surgery.

#Facelift #PreservationFacelift #DrSadati #PlasticSurgeryDay One After Surgery | Post-Op Update with Dr. SadatiDr. Kevin Sadati2026-06-25 | This is one of the more complex cases I’ve had the privilege to care for!
At age 54, she sought treatment simply to appear as rested as she felt. But her facial appearance was far more complicated. A decades-old accident caused a chin fracture requiring 18 separate operations for reconstruction, ultimately resulting in bone loss in that area. Years of Radiesse injections into the face and midface led to extensive scar tissue development beneath the skin, which markedly increased the difficulty of surgical treatment because we were operating around soft tissues rather than simply elevating them.
To manage all these problems, I performed a preservation deep plane facelift and neck lift, restoration of lost projection with a chin implant, augmentation of soft-tissue volume at appropriate sites with fat grafting and finally resurfacing and rejuvenation of the skin with a chemical peel.
This is only 1 day after surgery, but the degree of change is already apparent. Results at 3 months, when all effects are completely established, will be posted shortly. Finally, I am profoundly grateful for her willingness to share this experience with you. Reestablishment of her sense of self with feelings of confidence and well being.

When Cat came to see me, she was concerned about facial aging, neck laxity, brow descent, and changes in her upper lip and skin quality. To address these concerns, I developed a comprehensive surgical plan focused on restoring youthful facial proportions and natural-looking definition.
Today, at 6 months post-op, she is enjoying a refreshed, balanced appearance that better reflects how she feels inside. Her results have matured beautifully, and subtle refinements will continue as healing progresses.
One of the most rewarding aspects of facial rejuvenation is seeing patients regain confidence while still looking completely like themselves.
