Fauquier ENTVideo describes how the inner ear balance system works. The semicircular canals are shown along with corresponding head movements. PLEASE NOTE that this video is a simplification of what actually happens as well as angles used. For example, the video implies that the posterior semicircular canal is oriented perfectly between the ears and that head tilting ONLY stimulates the posterior canal. That is not technically true. The posterior (and superior) canals are actually angled 45 degrees from that shown in the video. Also the head tilt and nod actually stimulates BOTH these canals. But for the purposes of lay audience education, these complicating details were ignored.
Dix-Hallpike maneuver is also shown at the end.

Video created by Dr. Christopher Chang: https://www.FauquierENT.netTongue Scraping: Why and How?Fauquier ENT2022-11-10 | Everyone understands the importance of brushing and flossing the teeth to maintain good oral hygiene. However, for some patients, tongue scraping may be additionally required, especially if the tongue develops an unwanted exudate or bad breath is present.
A normal tongue is like a frilly carpet found in a home. And just like a carpet requires a good vacuum to remove accumulated debris, the tongue can also accumulate particulates that can’t be just rinsed away. Such particulates include fungus, bacteria, food particulates, and even salivary slime.
The only way to effectively remove such tongue debris deep down against the surface is by scraping it.

Having a phlegmy throat is a very common complaint. Although there are many potential causes of throat mucus including reflux, post-nasal drip, esophageal obstruction, allergies, and dysmotility. Interestingly, dairy has also been found to be a common trigger.
One explanation of how milk and other dairy products can trigger a phlegmy throat is due to its ability to curdle or thicken in the presence of acid. Acidifying milk causes milk proteins, like casein, to unwind and unfold in a process known as protein denaturing. The unfolded proteins are then free to interact with each other and clump together in a way they could not do when they were properly folded. The milk takes on a curdled appearance from the lumps of proteins that are binding one another.
Given acid reflux is a common medical problem, it is not surprising that so many people complain of throat phlegm after ingesting dairy products. When reflux reaches the throat, this condition is called laryngopharyngeal reflux or LPR rather than gastroesophageal reflux or GERD.
Keep in mind that one does NOT have to suffer symptoms of heartburn or a burning throat to suffer from reflux (condition known as "silent reflux").
As such, one of the first things that can be tried when a phlegmy throat complaint is present is to take an acid reducer medication and avoid all dairy products for at least 3 weeks.

Inside a glass bottle which represents the stomach, I already prefilled it with white vinegar heated to 98 degrees. The white vinegar itself with a pH of 4 represents the stomach acid. I then squirted the alginate product (0:23) into the glass bottle a bit over the recommended 1 teaspoon. Almost immediately, the alginate raft developed and floated to the surface!
The same experiment was then performed in water (1:26)... The results may surprise you!

REFLUXRAFT is derived from seaweed with added organic ginger and licorice root nutraceuticals. When taken after meals, it coats and protects the esophagus and stomach lining. More importantly, it creates a floating raft on top of the stomach contents. This raft physically prevents reflux from happening by plugging the opening into the esophagus whenever a reflux event is about to occur. RefluxRaft is designed to prevent backflow into the esophagus while providing nutraceutical benefits to the gastrointestinal tract.

#earlobegauge #plasticsurgery #medicalanimation #shortsBleach Baths for Recurrent Skin InfectionsFauquier ENT2022-10-31 | For individuals who suffer from severe eczema, folliculitis, impetigo, hidradenitis, boils, and/or skin infections... bleach baths may be very helpful to help resolve these skin conditions. Keep in mind that bleach baths is no different than chlorinated water in a swimming pool. Bleach is simply chlorine! At the strengths that are recommended, a bleach bath is simply like a double strength chlorine swimming pool. Just like a swimming pool, it is not a problem if small amounts are ingested or get into eyes. And just like how chlorine in a swimming pool can decrease the overall bacterial load in the pool water, bleach baths can similarly decrease the overall bacterial load on the skin that keeps getting infected.
Steps are as follows: 1. Start by adding lukewarm water to fill a tub for a normal bath (about 40 gallons).
2. Put 1/4 cup of common liquid bleach (for example, Clorox) into the bath water if tub is 1/2 full. It would be double that if the tub is full. Check the bleach bottle to make sure that the concentration of bleach (also known as sodium hypochlorite) is about 6 percent.
3. Completely mix the added bleach in the water. This should create a solution of diluted bleach (about 0.005 percent), which is just a little stronger than chlorinated swimming pool water.
4. Soak in the chlorinated water for about 10 minutes.
5. Thoroughly rinse the skin clear with lukewarm, fresh water at the end of the bleach bath.
6. As soon as you’re finished rinsing off, pat dry. Do not rub dry, as this is the same as scratching!
7. Immediately apply any prescribed medication and/or emollients.
8. Repeat bleach baths two to three times a week or as prescribed by the physician.
Do NOT use undiluted bleach directly on the skin. Even diluted bleach baths can potentially cause dryness and/or irritation.
Do NOT use bleach baths if there are any breaks or open areas in the skin (may cause intense stinging and burning).
Do NOT use bleach baths if the individual has contact allergy to chlorine.

In this particular patient, impetigo was initially present involving the face, neck, legs, and arms. Video specifically focused on a cluster of lesions found on the back of the right thigh. It was treated with mupirocin and cephalexin, but the infection continued to get worse. Given the lack of improvement, cultures were obtained from the oozing sores which revealed a multi-drug resistant staph infection. By the time this information was finally obtained, cellulitis was present given the surrounding redness and induration. Appropriate culture-directed antibiotics was immediately started along with steroids to help with inflammation. Immediate improvement was seen within 24 hours.
As the infection resolved, the affected skin takes on a more bruised appearance. After oral antibiotics were completed, topical skin care alone was pursued until the skin completely healed. Ultimately, it took nearly a month for the skin to return back to normal.
Hopefully this 30 day time-lapse showing how the skin looks from from initial onset, to worsening, to finally resolution will help patients (and parents) understand visually the overall healing time course.
There is no single "right" way to treat such skin infections. But what was used successfully in the care of this particular patient (that is available over-the-counter, excluding prescriptions) included the following:

When taken after meals, it coats the esophagus and stomach lining, but more importantly, it creates a floating gelatinous raft on top of the stomach secretions. This raft physically prevents reflux from happening by plugging the opening into the esophagus whenever a reflux event is about to occur.
Standard medications to treat reflux include H2 blockers (peptcid, zantac) and proton pump inhibitors (prilosec, nexium, prevacid, protonix), but long-term use is typically not recommended since these medications can cause unacceptable side effects. Unlike H2 blockers and proton pump inhibitors, alginate does not alter how the stomach lining itself functions.

#shortsEye Floaters: What Are They & What Causes Them?Fauquier ENT2022-08-14 | Visual floaters are objects that drift around in the field of vision. They may look like little blobs or cobwebs that shift around with eye movement and is impossible to focus on. Floaters are in fact particles suspended inside the vitreous body, the gel-like structure that fills the eyeball's inner cavity.
Please note that Dr. Chang does not evaluate eye issues like floaters. Video created for educational purposes. https://www.FauquierENT.net

Such patients often find themselves mouth-breathing. Normally, the nasal passages should be widely patent allowing for easy nasal airflow. Although there are many different reasons for nasal obstruction whether due to allergies or a deviated septum, large inferior turbinates or turbinate hypertrophy is a very common cause of nasal blockage. Turbinate hypertrophy can vary in severity leading to partial or even complete nasal obstruction if they become very enlarged.

#turbinatehypertrophy #nasalobstruction #hypertrophicturbinates #inferiorturbinatesCataract Surgery AnimationFauquier ENT2022-08-13 | Good vision requires a clear window from the outside world into the back of the eye. This requires not only a clear cornea which is the transparent, dome-shaped surface of the eye where light enters, but also a clear lens which is found behind the iris and pupil. Known as cataracts, when the lens become cloudy, overall vision quality deteriorates due to light being blocked. Initially, cataracts can be managed with stronger lighting and eyeglasses, but if the impaired vision becomes bad enough, cataract surgery can be pursued during which the cloudy lens is replaced with an artificial lens.
Keep in mind that the surgical steps have been simplified for a patient audience.

#cataract #eyesurgery #cataractsurgerySelective Cornea Transplant - Endothelial Keratoplasty (DMEK, DSEK)Fauquier ENT2022-08-11 | The cornea is the transparent, dome-shaped surface of the eye where light enters. It is a large part of the eye's ability to see clearly. Unfortunately for some patients, the inner lining of the cornea may become cloudy leading to vision loss, most often from Fuch’s Dystrophy or Bullous Keratopathy. Treatment is often pursued before the cornea becomes completely clouded. As such, treatment to restore vision requires a highly selective cornea transplant involving removal and replacement of just the diseased inner lining of the cornea with a donor graft, either DSEK (Descemet’s Stripping Endothelial Keratoplasty) or DMEK (Descemet’s Membrane Endothelial Keratoplasty). The main difference between DSEK and DMEK is the thickness of the inner lining removed and graft that is transplanted... 70-120 microns (DSEK) vs 5-10 microns (DMEK).
Keep in mind that the surgical steps have been simplified for a patient audience.

It is a large part of the eye's ability to see clearly. A cornea transplant, or keratoplasty, is most often used to restore vision to a person with a damaged cornea causing vision loss. Demonstrated in this animation will be a complete full-thickness cornea transplant, also known as penetrating keratoplasty.

Should medical management not sufficiently resolve turbinate hypertrophy, the overall size can be decreased with surgical intervention. Demonstrated here is a technique known as coblation which can reduce the size of the turbinates in a minimally invasive manner.

#nasalclogging #allergies #nasalobstruction #noseclogged #cloggednose #cantbreath #mouthbreathingCauses of Chronic Sinusitis (Anatomic Factors, Biofilm, Abnormal Microbiome)Fauquier ENT2022-08-04 | Normal healthy sinuses connect to the nasal passages in a complex network of air flow and drainage passages. Recurrent or chronic sinusitis may occur due to anatomic blockages such as a deviated septum, nasal polyps, or blocked sinus openings (0:14). However, persistent sinusitis may also be due to issues related to the sinus mucosal lining itself whether abnormal sinus microbiome, biofilm formation, and/or dysfunctional or damaged sinus mucosal cilia.
