Here is the case of a patient who came for a second opinion at 8 weeks post-injury, having already been under the excellent care of a face-to-face Physiotherapist 💪

The patients were placed in a weight-bearing brace, taken through Physiotherapy and a follow-up arthroscopy was performed after a 3-month period.
Results indicated that 69% of the lateral menisci healed completely and 18% healed partially, whereas 58% of the medial menisci healed completely and none healed partially. 20 of 25 anterior cruciate ligaments and 3 of 7 posterior cruciate ligaments healed satisfactorily.
This study indicated that acute tears of the meniscus, even when they occur in association with a cruciate ligament injury, can heal morphologically with nonsurgical treatment.
Obviously working as a Specialist Physiotherapist both face-to-face and virtually, initial and repeat arthroscopy is not feasible, so we can use a pragmatic combination of:
✅️ The mechanism of injury ✅️ Initial MRI scan ✅️ Bracing protocol and rehabilitation ✅️ Follow-up MRI ✅️ 3-month passive ligamentous testing ✅️ Patient functional status, signs and symptoms ✅️ Current best healing studies
to determine the quality of non-surgical success.

Currently radiologically classified grade 3 or complete ruptures may be more likely to heal when the torn ends remain within the intercondylar line, with migration (e.g., horizontalization) reducing the likelihood of success, although retracted ends can heal.

As a primary-contact clinician seeing patients with this injury every day, it is important bracing protocols and/or rehabilitation procedures are administered as soon as possible from the injury, in order to approximate the 2 torn ends of the ligament, improve the quality of the heal and mitigate the risk of secondary injuries.
