Oh my god, I did it! This was so goddamn hard. I had some very clean dodges in phase 2 with my saw cleaver. I am not sure if I should be happy, because I will do it with 35% less damage next (NG+6 No Gems/Sprint). It will be the hardest Bloodborne/Dark Souls kill ever done, even harder than my +0 No Sprint Maria fight which is the current best kill.
