It also features three new campaigns: one focussing on ancient Rome, one set in the Pacific theatre of World War II, and telling the story of the Kwan Do family, a Chinese dynasty involved in the future colonisation of Mars.

Wolfenstein has a number of features popular at the time that distinguish it from its predecessors, such as regenerating health, upgradable weapons, and a hub world that connects the game's various levels. The medallion is connected to a parallel world known as the Black Sun Dimension, which the Nazis want to utilise a source of power. Like weapons, the medallion can be upgraded.

The campaign consist of two sets of six missions and takes place after a Soviet victory in World War III: the USSR has established world domination, and Yuri has been destroyed - though his brain has been preserved, and his followers are still at large.
