[CVR] Blue Panorama Flight 1504

Accident Aviation 2022-01-10 | 4 kts before V1 the right engine exploded. The F/O was flying. The Captain elected to continue the take-off and, after completing the emergency actions, the fire was still uncontained. There was no possibility to dump fuel, of course, so the Captain took the controls and landed the aircraft 42 tons over the max landing weight. They took off from runway 16R but the Captain decided to land on 34L because the fire was in the right engine and the wind from the west would have kept it away from the cabin. The final speed was 179Kts, flaps 19. After the aircraft stopped, of course, most of the tires deflated. The 287 pax were evacuated with no injuries and the airplane suffered no damages, apart from the right engine. The enquiry established that the blast and fire occurred because of a leaking high-pressure fuel hose. Apparently, over 700 kgs of fuel had leaked into the nacelle during taxi. It ignited during take-off.

American pop singer Ricky Nelson and six others—including several band members and his girlfriend—were killed in the accident. Both pilots survived. [ATC] 2015 Beechcraft Bonanza N5626D crash

Accident Aviation 2021-12-29 | The commercial pilot was conducting a cross-country personal flight. About 15 miles from the destination airport, while in instrument meteorological conditions at 3,300 ft mean sea level, the pilot declared an emergency to air traffic control, stating that the airplane was experiencing an "engine problem." The pilot asked the controller about the nearest airport, and the controller provided him with radar vectors to that airport. The pilot then attempted to glide the airplane to the airport, but he reported that he was unable to maintain altitude. The controller then advised that there was a highway right of the airplane's position and about 2.5 miles away, and the pilot responded, "we have no engine we're [in instrument meteorological conditions] I need help." The airplane impacted a house before reaching the highway. It is likely that low cloud ceilings prevented the pilot from locating and navigating to a suitable forced landing site.

Accident Aviation 2021-12-28 | A Learjet 35A crashed in a residential area, killing all on board. The aircraft was completing the turn for the final approach to runway 27R at San Diego-Gillespie Field, California. It struck Pepper Drive in a residential area of El Cajon, San Diego, 2,6 km east of the runway threshold and burst into flames. It is unclear at this time how many occupants were on the plane. At 19:08 Gillespie Tower cleared the flight to land on runway 17. At 19:11 the flight contacted Gillespie Tower, cancelling their IFR clearance and requesting to land on runway 27R. This was approved and the Tower controller cleared the flight for a left hand traffic pattern for runway 27R. The flight crew then requested the runway lights to be turned up, to which the controller replied that "they are at a 100% now".

Accident Aviation 2021-12-20 | On April 6, 1993, China Southern Airlines Flight CZ3157 (model B757-200) was hijacked by hijackers Liu Baocai and Huang Shugang. The hijacked plane landed at Taiwan Taoyuan International Airport at 10 am that day without casualties. It flew to Guangzhou Baiyun Airport at 3:40 pm that day and landed safely. Accident Aviation 2021-12-16 | American Eagle flight AA4439 returned to Atlanta Hartsfield-Jackson International Airport, Georgia, USA, after suffering severe controllability issues after takeoff. After reporting 21 DME South of the airport, the flight was cleared down to 3000 feet. They were told to expect a clearance to turn finals for runway 02 after passing 4000 feet. At 06:10 UTC the crew reported at 4000 feet at 29 DME South of the airport. The controller then stated: "You are cleared to ... join final for runway zero two, descend to two thousand five hundred." This was the last contact with the flight. One of the flight crew members then remarked: "We’re getting pretty close to the hills here." Shortly after deciding to initiate a climb, the aircraft impacted a 4100 feet high hill just below the top.

Accident Aviation 2021-12-08 | On 23 June 2010, the Beechcraft A100 King Air (registration C-FGIN, serial number B-164) operated by Aéropro (2550-4330 Québec Inc.) as flight APO201 was making an instrument flight rules flight from Québec to Sept-Îles, Quebec. At 0557 Eastern Daylight Time, the crew started its take-off run on Runway 30 at the Québec/Jean Lesage International Airport; 68 seconds later, the co-pilot informed the airport controller that there was a problem with the right engine and that they would be returning to land on Runway 30. Shortly thereafter, the co-pilot requested aircraft rescue and fire-fighting (ARFF) services and informed the tower that the aircraft could no longer climb. A few seconds later, the aircraft struck the ground 1.5 nautical miles from the end of Runway 30. The aircraft continued its travel for 115 feet before striking a berm. The aircraft broke up and caught fire, coming to rest on its back 58 feet further on. The 2 crew members and 5 passengers died in the accident. No signal was received from the emergency locator transmitter (ELT).

Accident Aviation 2021-12-07 | An Air France Boeing 747-400, registration F-GITE performing flight AF-733 from Santo Domingo (Dominican Republic) to Paris Charles de Gaulle (France) with 142 passengers and 14 crew, was enroute at FL390 over the Atlantic Ocean about 400nm southwest of Shannon when the crew received a forward cargo smoke indication and decided to divert to Shannon (Ireland). The crew instructed emergency services to not open the forward cargo door before all passengers had left the aircraft. The aircraft landed safely on runway 24 about 70 minutes after the emergency call and taxied to the apron with emergency services following the aircraft. Emergency services did not find any trace of fire, smoke or heat.

[ATC] 2014 Ukraine Air Force 80 Yellow shoot down

Accident Aviation 2021-12-02 | An Antonov 30 plane of the Ukraine Air Force crashed after being hit by ground fire near Slavyansk. Photos show the right hand engine on fire and some crew members bailing out. Pro-Russian sources reported that the airplane involved was an Antonov 30 reconnaissance plane, while Pravda Ukraine quoted Ukrainian reports saying the airplane was an Antonov 26 carrying humanitarian cargo. Unconfirmed reports indicate that An-30 coded '80 yellow' was involved.
