We all know that Vitiligo is a skin disease which is characterized by white skin or white patches on the skin. But there are two types of vitiligo. One which we call as a segmental vitiligo and one which we call as a generalized vitiligo. In segmental vitiligo the vitiligo is limited to one area of the body, maybe the trunk or the arm or the leg and in generalized vitiligo. There is a bilaterally symmetrical distribution of the vitiligo patches meaning on both sides of the body these patches may be distributed the difference between segmental Vitiligo and generalized Vitiligo is that segmental Vitiligo usually does not spread very much beyond that segment where it is localized but the disadvantage is that it is poorly responsive to treatment and has to usually be treated with surgical methods or laser treatment whereas in generalized Vitiligo it could affect all parts of the body and it usually responds well to treatment. Now this differentiation of Vito is basically important for the dermatologist in order to understand what are the treatment modalities to be used . For the patient it is not so much of a differentiating factor they need to anyway follow the routines which are being prescribed by the dermatologist. 25th June is World Vitiligo Day and we are trying to create an awareness about vitiligo. How is rectal prolapse treatment done? So what is rectal prolapse? Let us understand. Rectum is the last part of large intestine. It's about a 15 cm long tube and below that will be anal canal which is about 3 to 4 cm long tube. Anal canal will be normally closed and rectum will be fixed to the what we call a sacrum or a bone behind it and motion passes through the rectum into the anal canal and comes out through the anal canal. When the sphincters open sometimes this rectum becomes lacks and the attachments to the sacrum and other ligament will become loose. That whole rectum itself will start projecting outside through the anal canal . So then that is called rectal prolapse. In that there are different stages. Minimal prolapse will be there and there may be very big prolapse. So sometimes we see whole rectum and sigmoid colon itself will be coming out pushing it back and managing for years. I have seen a patient managing over 60 years from childhood pushing it back whenever it comes out. So now what is the treatment so treat treatment of prolapse is only surgical because it is a surgical issue it is mechanical issue it has to be corrected mechanically so if it is very mild prolapse a small procedures are there called deror procedure while we remove a part of the lining membrane and fix it back again into the rectum. So if it is not a major prolapse we can even fix. There is something called more procedure where we fix it to the sacrum through the anal canal. A stitch goes through and through the rectum and through the fascia covering the sacrum and fix it that is one more procedure developed by Indian surgeons. And then regular surgeries is through laparoscopy or laprotomy by opening the abdomen or through laparoscopy. They pull the rectum which has come out and then put a mesh between the rectum and the sacrum and fix it to the sacrum so that it will not come out. That is called rectopexy. One more procedure what we normally do is if it is too big a prolapse from outside only will resect out completely remove whatever is prolapsing and again anastomoses are join that to the anal canal at the same time. If there are any defects in the sphicnter muscles there are something called levators which are stretched all that can be repaired at the same time. So there are different approaches. We normally do from below. We don't open the abdomen and do the procedures for rectal prolapse.

Laparoscopy or minimal invasive surgery. So laparoscope is a very tiny instrument with 10 mm diameter and it has attached light and camera. With the help of laparoscope we can actually see inside of abdomen we can also see the pelvic organs. So we make a small incision near the umbilicus 1 cm and through this small incision we introduce laparoscope inside the abdominal cavity and we can clearly visualize the pelvic organs like uterus ovaries and tubes so in the same sitting we can also do the fertility enhancing surgeries. For pregnancy to happen tubes and ovary relationship really matters because once ovulation happens to they capture the ovum into fimbria and then into the ampula and where sperm travels from cervix to the tubes and where the fertilization happens and tube should be healthy for this fertilization to happen and after staying in tube for 2 to 3 days and implants in the uterus. So for pregnancy to happen tubes should be healthy ovaries should be healthy and uterine environment should also be healthy. Through laparoscopy we can actually study this tubo ovarian relationship. If there are any adhesions peritubal adhesions in the same sitting we can do the adhesions and we can releasee the fimbria so that when ovulation happens they can pick up the oven to the tube. If there are any cyst in the ovaries if there is any big large simple cyst more than 5 cm or in mild endometrosis. When there could be chocolate cyst in the ovaries in the same sitting we can do the ovarian cystectomy. Now enough enough evidence says in the mild endometriosis laproscopy really helps and such patients after laproscopy they can even conceive naturally. So after these fertility enhancing surgeries 6 months are best after post surgery to conceive naturally or with minimal treatment like ovulation induction or IUI.

Joint pains is a very broad term it's very broad term we've got joints all over the world. Ioints are those spaces where two bones are two different areas are coming from together and they will have flexibility. Human papiloma virus has about 100 genotypes. Most of the Indian women cervical cancer is caused by genotypes 16 and 18. These are the most common cause of other cancers like anal cancers, vulval cancers vaginal cancers. HPV 6 and 11 are known to cause genital vs HPV vaccines. There are three types of vaccine balent vaccine quadrivalent vaccine and nonavalent vaccine these are known to prevent this carcinoma with 99% efficacy. Bivalent vaccine gives protection against HPV 16 and 18 quadrivalent vaccine gives protection against genotype 6 11 16 and 18. The newly introduced nonavalent vaccine that is it gives protection against nine genotypes of the HPV which are both oncogenic and non-oncogenic previously these vaccines were advised only for girls age between 9 to 14 years and catch up after 14 to 45 years but now it is advised even for men or the boys age through 9 to 45 years because they're known to prevent penile carcinomas enal carcinomas as well as oro fecal carcinomas. So what is the dosage and schedule? It is given 5 ml IM . The first dose is given through 9 to 14 years and it is repeated after 6 months. So let's understand the process of aging . Bfore we try and understand why do we get wrinkles. So aging is a complex interrelated 3D process which affects multiple organs starting from the bone the muscle . Te fat as well as the skin now what happens to the bone. Let's begin with the bones bones because of ageing. Start getting resorbed or they start getting absorbed into the system. For example if our eye sockets are here the sockets start enlarging because the bone gets resolved that causes the eye to sunk in the eyes look smaller and that's when you start developing the under eye lines and wrinkles also if you look at the chin the chin starts start resorbing or going backwards and become shorter. So the skin starts hanging and that's when you notice sagging of skins and lines over here then after the bone comes the muscle now repeated activity of the muscles over the years changes the muscles tone that's when you start developing dynamic wrinkles. Dynamic wrinkles are wrinkles when you express on your face. Dynamic wrinkles develop when you actually start using these muscles so when you start frowning is when you see the frown muscles when you smile you start getting the crows feet this is because of the muscle activity over the years and with aging the skin over the muscle also changes then the skin starts dipping into the fold causing a static wrinkle or a static line further comes the fat now fat also starts getting resorbed and fat starts moving downwards because of gravity so you notice sagging you notice jowls and many of times there is hollowness over here because the fat volume starts reducing and last is our skin especially in the second layer of the skin which is called the dermis. It contains three very important ingredients collagen elastin and hyaluronic acid. All these three form of structure gives support as well as bounds to our skin now once they start getting deteriorated because of age because of sun exposure collagen lasting production reduces that's when the skin becomes thinner and starts sagging. That's when you notice more lines and wrinkles developing on your face so this is how a wrinkle is generated into our skin aging right from the bone to the skin now how do we treat these wrinkles there are multiple ways that we can do right from applications to surgical procedures but somewhere in between come the injectables which are which are now getting very popular. There are two ways we inject one the injectables that target the muscle. So when the muscle movement is more you notice that the skin dips in the area where the muscle moves so we stop the movement of the muscle that's why we use something called the botolinum toxin. This is derived from a bacterium it is a protein so when this is injected into the muscle the activity of the muscle starts reducing so even when you express the muscle doesn't move the skin on top doesn't change that is how you can prevent as well as reduce the wrinkle appearance. The second form of injection is where you fill in the fat or the collagen which has depleted due to age we fill that up like we fill cotton in a pillow. So when you fill that up the skin gets lifted as well as the volume comes back to the face now these are called dermal fillers there can be multiple variety of these dermal fillers the commonest that we all use in India is either hyaluronic acid or fat injections there are others that are available too which promote collagen production. Now when we inject a filler we injected bases where we need it so deeper fillers are injected onto the bone superficial fillers are injected in and around the skin. Generally if it is a good brand starts with almost 15,000 and depending on the number of injections that you do it can go all the way up to a lack. But remember all these treatments are temporary. Botulinum toxin last for about 6 months and the fillers generally last for a year to a year and a half. So you need to repeat these procedures accordingly and your skin does not get affected and there are no other side effects if injected by the right person. Stasis eczema also like a person who has a varicose veins, the stasis eczema or stasis dermatitis both are same commonly seen in people with varicose veins or any damage to these vasculature to the lower limbs commonly seen in the lower limbs. Now when there is varicose veins or the vessel wall is weakened the pumping action of the blood back to the heart is reduced. So there's a backflow of the blood and this when it back flows the little seepage of the blood particles comes and lies from the vasculature they come and lie under the skin giving it a pigmented appearance, itchy, dry, scaly, and if it gets infected it becomes erythemaous is and swollen and painful also. So the treatment to this is treat the underlying that is the vascular deformities varicose veins or anything like that has to be addressed first. Cataract, what is cataract? In the eye there is a natural lens which we call as human lens. The lens is made of proteins this lens helps in focusing the ray of light from the outside source onto the retina. The retina then converts the rays of light into electrical signals and this electrical signals are transmitted by the optic nerve to the brain. So the eye is basically a receiving station the brain is the main processor and we see through the eye . When the human lens becomes opaque we call this condition as cataract cataract is the most common cause of reversible blindness. Cataract occurs because of aging process just like the hair on our head turns gray as we start getting old so does the lens inside the eye which is made up of proteins also turns gray. This is a gradual process so it passes through various stages. You may have what is called as a nuclear cataract where the center part of the lens gets affected or it is called as a cortical cataract where the peripheral part of the lens gets affected by cataract. When the cataract is not treated it passes through various stages we call this nuclear catarct can be graded according to the density or hardness as grade 1 2 3 4 which depends upon the color and the hardness of the nucleus. Cortical cataract also passes through various stages. Initially it must be a minimal cortical cataract as the cortical cataract progresses it may lead to a condition what is called as a mature cataract. If this mature cataract is not operated it can lead to what is called as a hypermature cra what happens in hyper mature cataract is the cortex starts becoming liquefied and the nucleus that is present in the center start shrinking if here again the Cataract is not operated liquefied cataractstarts leaking out of the cataract lens and this leads to a condition what is called as glaucoma or what is called as lens induced glaucoma. So hypermature cataract is an complication of not having undergone cataract surgery which leads to complications which can lead to what is called lens induced glaucoma. Lens induced glaucoma causes severe pain redness watering and as the pressure remains high. It can damage the optic nerve which carries the impulses to the brain and even it can lead to irreversible damage or loss of vision to the eye. So cataract as I told you first is the most common cause of reversible blindness. If this cataract leads to hypermature cataract it can lead to irreversible blindness. So one should not allow the Cataract to the reach a stage of hypermaturity and go to this complications. The decrease in vision is the important symptom of any cataract. So if there's a decrease in vision kindly get your eye examined by an ophthalmologist.

There are various factors which can increase the chances of hypertension in pregnancy like the extremes of reproductive ages like in teenage pregnancy or in mothers who are early pregnant. Similarly if obese patient they have more chances of hypertension in pregnancy. The patients who is already hypertensive or who has a kidney disease have high chances of high blood pressure in pregnancy. Similarly diabetic patients have high chance of high blood pressure in pregnancy. So these are few risk factors if patient had hypertension in previous pregnancy then they also have high chances of high blood pressure in this pregnancy. Similarly if patient has family history of hypertension or pregnancy induced hypertension in mother or sisters then she will also have higher chances of pregnancy induced hypertension. So these are the few risk factors for the PIH. This high blood pressure in pregnancy is again a problematic condition and we are always afraid of that's why in every antenal visit we do checkup for blood pressure in pregnant girls. This high blood pressure affects the health of mother and baby both. In baby the moment the blood pressure rises in the mother the blood supply to the baby decreases that's why the growth of the baby is impaired and fluid around the baby also decreases and when the blood supply of the baby is decreases too much the heartbeat of the baby also becomes abnormal and we have to do preum delivery to save the life of the baby. So chances of pre-term delivery also increases. If the observation ation or the treatment was not done properly sometimes it causes the death of the baby inside the uterus. This high blood pressure if not controlled properly it can cause separation of placenta, heavy bleeding during pregnancy which is very harmful to the baby. For mother this high blood pressure can affect any organ of the mother which includes brain, it causes convulsions, brain edema, brain hemorrhage, it affects heart, it affects liver and it can damage kidney also. So as a whole body is affected by this high blood pressure of the mother during pregnancy.

Endoscopy is a very commonly used diagnostic tool to see inside the patient's GIT. That is the gastrointestinal tract . So these endoscopies can be of broadly two types that is an upper GI endoscopy wherein we introduce the camera for the mouth of the patient. We see the upper GI tract that consists of the oral cavity the food pipe and the stomach. The second type of endoscopy is the colonoscopy wherein the camera is introduced through the anal canal of the patient and the entire large intestine and the last portion of the small intestine is seen . So who needs an endoscopy? So whenever the surgeon or the physician feels that there could be some disease inside the GI tract which needs to be seen then an endoscopy will be required to be done. So during endoscopy what should the patient expect? So during an upper GI endoscopy there could be mild amount of discomfort initially the endoscopist will give you an anesthetic spray to your throat and then the camera is introduced into the oral cavity the endoscopist will ask you to swallow the scope and the scope will go inside if some abnormality is found inside the GI tract then a biopsy could be taken to take a sample of the problematic area and send it to the lab for testing if there is no problem the endoscopist will take a couple of photos and the camera is brought out the entire procedure for the upper J endoscope will take less than about 3 to 5 minutes and it is relatively comfortable after the endoscope is done. So coming to the colonoscopy so in this colonoscopy the camera is introduced through the anal canal and as the camera advances the intestine is filled up with air due to this the patient will experience some amount of bloating sensation of the abdominal cavity and during the passage of the scope through the intestine at a couple of places there could be some pain. So generally the colonoscopy is done under sedation so that the discomfort is minimized. So even in this colonoscopy if the any abnormality is found we might take a biopsy to send to the lab for testing or if there is no abnormality photos will be taken and this camera is brought out of the intestine. So this is in brief about what is done and what should you expect during endoscopy.

So today is your third day after hair transplantation the third posttop day. So what are you supposed to do today? You have to keep spraying a normal saline spray which we have provided to you every 1 hour till you sleep. You can avoid doing spray at night only during the day you have to do it every 1 hour you should not touch the recipient area today the donor area from the back of scalp or the beard area or the body if it is done then this has to be cleaned with the mild shampoo watch we have baby shampoo you can use to clean the area. Tap and dry the area and leave it open apply an antibiotic ointment on the donor area. The recipient area should not be touched continue with all the medications what we have given antibiotics painkillers and ointments tablets anti-d what we have given continue everything the recipient area will have brown crust which is visible this will be visible like this for the next 2 weeks. If you have to go out you can start going out from today. But don't stand out in sunlight for hours together just go out sit in a cab and travel. Long distance travel is fine no need to be concerned you can wear a loose fitting cap like a baseball cap should not be hugging very tight. Skull cap should not be used you can't wear a helmet for almost a month. You can't do any exercise intense workouts or pumping exercises blood should not happen. So don't do any vigorous exercise or walking also. Casual walking can be started in the evening or early mornings where peak our sunlight should not be exposed you can't stand out in sunlight for hours together it should be only for few minutes if you want to travel. Otherwise if you have opportunity of working from home you can continue working from home. You should not sleep on your face down at night as it may cause more swelling. There's a possibility on the third or fourth day you can start seeing swelling on the forehead which can trickle down around your eyes and there may be some swelling in some patients we have very minimal swelling which will reduce with the medication already we have provided in some patients the swelling can be more based on their health condition . But it is nothing to be worried about this is just a fluid collection and plasma collection which is happening under the skin and it will not cause you any side effect even if the swelling is there around your eyes it will not cause any issues with the eyes there is no blindness or eye problem will not happen. So nothing to be worried about the swelling will go by itself you can apply some aloe vera gel and gently massage from the center to the outright direction. If your swelling is more you just sleep on your back and just let the fluid drain for half an hour 1 hour if you do that in between two to three times a day the swelling will come down by itself if required we will suggest you some medications also if the swelling is more. So sleeping position is also important on this day. For the next few days don't sleep on the sides if you have done transplant land on the temporal area or the side burns you can sleep on your back if the frontal area is done you can sleep on the sides if only the frontal area is done the vertex is done where the crown area transplant is done then sleep on your sides you cannot sleep on your back because this hairs which is transplanted which may touch the pillow and the hair follicles may come out and hair will not grow in the area. So for the next 10 to 15 days you have to avoid sleeping on your back if you have done on vertex area and on the sides if you have done on the temporal area. So the position is can be fixed if you are using long bolster like pillows on the sides if you're sleeping on on sides at night. So that you don't accidentally turn if you're sleeping on your right side keep the pillow on your left side supporting your back if you sleeping on your left side keep the pillow on your back on the right side so that you don't accidentally turn at night when you are sleeping. Brown crusts will be visible this can be there for 10 to 15 days. You can avoid smoking and avoid drinking don't have any heavy oily spicy foods for the next 4 to 5 days. It's 30% of the normal deliveries have cord around the neck cord around the neck is definitely not a contraindication for normal delivery definitely. The thought about cord around the neck is worrisome to many pregnant women in the later stages of pregnancy . Aound the neck is usually harmless and it is because of random movements of the baby or it's because of excessive long long cord or even because of excessive liqor around the baby . It is harmless and it does not pose any problem for vaginal delivery. Cord entanglement around the other parts of the body only cause fetal distress . In case of a good uterine contractions whenever the cord is compressed otherwise cord entanglement even around the other parts of the body around the trunk is harmless it doesn't matter whether it's a single loop of cord around the neck or double Loop or multiple loops. All patient can be delivered vaginally what are the complications of cord entanglement cord entanglement sometimes causes fetal distress there is compression on the cord due to labor utrine contractions in cases of short cord and if there is a cord on the neck it may prevent descent of the baby. Only in cases of fetal distress but the patients have to be taken up for a C-section otherwise all patients cord around the neck can be tried for vaginal delivery. There's no way to prevent cord entanglement. No sleeping positions which prevents cord entanglement as is generally thought of.

Restless leg syndrome is nothing but improper movements or what in the local language or normal language we'll call it as like unauthorized movements. There are two types of restless leg syndrome, like one is a primary RLS we'll call it as in secondary RLS. The primary RLS the cause mainly we don't know it can be a neurological disorder. It can be triggered due to any other reasons. The secondary RLS is can be caused mainly due to the iron deficiency anaemia or if the person is having any kidney related impairments or kidney diseases and some other metabolic disorders also can trigger this problem. Restless leg syndrome is nothing but disease we also call it as Willis- Ekbom disease. So usually it is characterized by improper movements or what we say unauthorized movements or we won't be knowing when we are doing movements. Basically it is a neurological disorder the patient sometimes will not be aware also the movements are happening in the leg and sometimes they will be knowing also which will not be under their control. During they are sitting at one place in the chair or during sleep or when they are on the bed some people usually say it increases over the night. Basically it is a neurological disorder so causes which mainly include as I said iron deficiency anaemia or if any iron deficiency if anybody is having if the hemoglobin is less than five or something like that or the serum ferritin levels if they are low the patient can develop this restless leg syndrome. That is a main cause and genetic predisposition there are certain categories of families or people they can easily develop this restless syndrome pregnancy also is one of the main cause for this resless leg syndrome usually it occurs mainly in third trimester. But not every pregnant people will get it there are certain people they can develop this restless leg syndrome and then other medications. There are certain medications like antipsychotic drugs and then there are medicines which have been taken for depression all medications also as a side effect this RLS can be caused so there are certain chronic diseases like kidney diseases and all also can cause this stress like know the main symptoms . Usually patient feels aching of legs itching over the legs or crawling feeling or tingling sensation in the legs during the night. Usually they feel it more and then it can cause uncontrollable movements or as I said unauthorized movements of your own legs without knowing to yourself also can happen. So these are the major symptom in this problem so how do we diagnose this RLS the medical history we have to observe the medical history of the patient when they are saying to us we have to clearly understand the history. That is one of the major important diagnosis tool for us. And other thing is the blood test as usual it is there we can find out the iron deficiency anemia or we can get it done the iron profile serum fertin levels iron levels all other blood tests. So physical examination in examining legs and other body parts also helps in diagnosing the leg syndrome study is of the study we'll call it as polysomnography which mainly helps in determining restless leg syndrome. I'll tell you how to you know how we'll treat this mainly the lifestyle changes doing the physical activity regularly doing 30 to 40 minutes walking per day taking regular physiotherapy sessions at your home or going to the physiotherapist which will help you in treating the disease certain medications will give the iron supplements which will be available in the market or even your doctor can provide or your nearest pharmacist can provide these tablets very easily and benzodiazepens. These are the drugs which will be available by the doctor's prescription and opioids or certain drugs which will help you in treating the problem so establishing regular sleep pattern will also help you in treating half of problem and avoid voiding certain drugs and such as caffine and avoid drinking outside beverages also will help you in regulating this disease. Basically this disease treatment is mainly treated by your physician that means like depend upon the symptoms or like how many times you are getting the episodes in the night depend upon that the treatment will be planned so you should consult your doctor for the same.

The combined First Trimester screening report gives us a risk value. It can be either high risk low risk or intermediate risk low risk means the risk is more than 1 in 1000. If it saves low risk that means the chances of your baby getting chromosomal problems is less like and when the risk is intermediate risk which is between 100 to 1000 we should test it further. With either NIPT or amniocentesis and if the risk shows as high risk that means which when it is less than one 100 we need to definitely go for invasive testing called amniocentesis. If the risk is high it means the probability of your baby getting chromosomal problem is high. So we need to confirm whether it is really an abnormality or if it is is a false positive. For that we need invasive testing either chorionic villus sampling or amniocentesis based on the period of gestation. If it crosses 16 weeks we go for amniocentesis and if it is less than 16 weeks we go for chorionic villus sampling and when the NIPT report shows it has low risk that means the chances of baby getting chromosomal problem is less likely and after the high risk report in the combined first trimester screening. If the NIPT shows it as low risk that means the probability is less likely if it again shows as high risk in NIPT we need to definitely confirm with invasive testing again.

Some cesareans are planned cesarean section maybe because they had so many cesareans before or they were told to have a cesarean for some reason these are planned or also called elective cesarian section. So the other types or the most common types of cesareans that we do are an emergency cesareans means we don't plan that cesarian we go thinking that women will have a normal delivery something or other changes and then the cesarean will become necessary for them. So they are called emergency cesarian sections so we have so many categories of cesarian sections also. Depending on how soon we have to do a cesarian we have categories like category one is the the cesarean is must to save the life of mother or the baby and it is so emergency that you have to have the baby out within 20 minutes. These are called category one cesareans that is an extreme emergency and we do drills to make sure that you know it can happen and the whole team is ready to do so. This is only when you think if the previous cesarean section is trying for a normal delivery and we think that car has ruptured then the baby can die very quickly. So this is only many times that we think minutes matter we do this category of cesareans. Sometimes a cord is a cord around the baby can come down the position of the baby is not right it's not head down it's across then the chances of this cord coming down with water break is high. So these are the times that we have to do in a very very emergency situation we call it as category 1. What is Category 2? When cesarean is necessary but it is not an emergency that we have to do in you know 10 - 20 minutes but it has to be done within that hour when we think baby is a little distressed but not so much that we have like you know the baby's heartbeat is down and not picking up. But baby's heartbeat is going down each time it's not correcting we think there is a problem with the baby. So we need to be a little hening things but not to an extent that you know everything is dropped down without consent we are going in wheeling straight to theater we call it. So usually we just done within 1 to 2 hours of decision these are category two what are category three cesarians which is it has to be done but to. So the time of women and the team we can plan like see we are planned to do aan but you come in an emergency with pains. So it has to be done but it is not an emergency baby is fine mother is fine but we have to do in that day. So around 6 hours or 8 hours we can plan based on various other things when you have had food to eat, you can wait till you are safe to have anesthesia and then get this done, we call it as category three. Category four is when you plan a cesarean you give a date you come and vision then we call it as category four. So this is categories of cesarean and now we have seen planned and unplanned emergency and elective cesarean. These are types of cesarean sections.

Topical warm oxygen therapy remains one of the most useful techniques in which we can try to heal the wounds that a patient has come to us with in a much better way. There are many augmentative therapies when it comes to healing of wounds in diabetic feet and topical warm oxygen therapy remains one of them. It is not the same as hyperbaric oxygen therapy. How is craniosynostosis diagnosed? Now most times the diagnosis is just clinical. What I mean when I say clinical is it is enough if we actually examine the child and also ask some history from the parents about how things have progressed since birth . In addition to our clinical examinations we still might have to do a few tests to confirm the diagnosis. The best test to confirm craniosynostosis is CT scan of the head with a 3D reconstruction of the CT scan. This gives us very detailed knowledge about how the anatomy of the bone is and how the sutures have fused According to which suture has been affected by craniosynostosis. Most of the single suture craniosynostosis babies will only require one CT scan to diagnose craniosynostosis conclusively. But sometimes in syndromic craniosynostosis or in multi- suture craniosynostosis we might also need an MRI scan to see how the brain is doing. The CT scan is very good for us to know about the bone . But MRI can give us good knowledge about the soft tissue that is the brain inside the skull. In addition to this we also do an eye checkup to see how the optic disc is because optic disc is like a window for us to see how the condition of the brain is. If there's a lot of pressure on the brain it shows up as papilloedema on the optic disc. So we do an eye test for every patient with craniosynostosis. Now sometimes we might have a very minor doubt or whether this is craniosynostosis or deformational plagiocephaly. In such scenarios we might only do a skull x-ray to confirm whether it is a craniosynostosis or it is a deformational plagiocephaly.

Vaginal beding in first trimester. Most of them come to us worrying asking whether I am ending up in a miscarriage because I'm having bleeding in the first trimester. But the thing to know is not all bleeding will lead to miscariage. So there are many reasons for the bleeding in the first trimester. Because your cervix and vagina are very sensitive even for the pelvic examination or the intercourse you might even bleed that's bleed on touch . The most common reason for vaginal bleeding in first trimester is the implantation bleeding because before the egg has been implanted into your uterus if there is any mobility that's where you will even see the vaginal bleeding. What exactly your doctor will advise when you have a vaginal beding they usually ask you to avoid lifting heavy weights and when you come visit your doctor they'll have an examination over you. They'll order few tests as well they are the ultrasound and even sometimes they might check with a doppler also for the baby's heartbeat. They make sure the baby is doing fine inside or there is any hemorrhage all the bleeding collected posterior to your placenta and they even check that so the Doppler that's a machine only used to see your baby's heartbeat. Usually it is carried out after 11 to 12 weeks of your gestation. What treatment will be given to me? There is no specific treatment that have been show proven to be effective in the case of vaginal bleeding. Usually to continue your pregnancy your doctor might start you on progesterones and if you're bleeding excessively that's where your doctor might even start you on something called as tranexamic acid also depending on the situation. Now the other reasons they come up and they ask is ma'am I'm having a bleeding is that my mistake or I'm going through a miscarriage is that something I've done which have lead to this condition absolutely not. Please don't blame yourself for anything which has happened nothing you know the activity or any foood will actually lead to any vaginal bleeding or abortions. To be frank most of the abortions which might happen in the first trimester it's a nature's way of calling. That is the abnormal babies are usually expelled in the first trimester. So it is not your fault it is just a nature.

The treatment of arthritis depends on the stage. So initial stages like grade 1 and grade two can be managed with physiotherapy medications. Cyanosis of hand and feet is medically known as acrocyanosis and just after few hours of birth. This is a normal finding because baby is in a transition phase and body is trying to basically conserve heat to the core of the body so that vital organs like kidney, brain, heart, lung they are getting the heat and they are compromising the peripheral temperature so that more vital organs are getting heat to basically function well. But after a few hours of birth if you feel that your baby is extremities are dusky that is acrocyanosis is there. That it can be like because of hypothermia. So first of all we have to sort out the temperature by that I mean change the temperature if you're using AC and it is quite chilling then you should turn it off and second thing is that you can increase the layer of the child and maybe you can add a cap mittens socks so that baby is warm. Second thing if you have already rewarmed the child and but still you feel that hands and feet that is extremities they still old on touching or they are still dusky in color. That means that you are dealing with some other serious things like infection, like hypoglycemia and maybe some critical congenital heart disease. These all three conditions are like life-threatening and you should immediately meet your doctor if you have not been able to rewarm your child even after increasing the layer increasing the temperature even after that if this acrocyanosis or bluish discoloration of hand and feet is still there. You should meet your doctor to rule out infection, critical congenital heart disease or hypoglycemia.

So facial spasms or facial tics, hemifacial spasms are mainly when the facial muscles are twitching. It generally tends when the entire one half of the face is involved. There can also be muscle twitching when only both the eyes are involved where it is called as blepharospasms. These are involuntary closures and they cannot be really controlled by the patient. It can be pretty disturbing especially when you're talking to somebody when they're thinking that you're winking or when there's a lot of disturbance because you cannot open when you want to open your eyes. So this is the main issue with blepharospasm and hemifacial spasms. So it generally starts with light twitching usually in the lower eyelid area and then it spreads to the entire eye surrounding and then when there's a hemifacial spasm it spreads next to the nose and the lips area where there's constant twitching. When the patient will feel that whenever he's trying to talk the muscle is going not in the way that he wants to or she wants to go when it's blepharospasm there's constant closing of the eyelids. Sometimes it starts with very few episodes and then it slowly starts increasing over a period of time where the frequency of closure keeps on increasing such that they'll be finding it difficult to open their eyes . It takes some time for the eyes to open. So hemifacial spasms again there's irritation of the nerves supplying the facial muscles it can be where there's a blood vessel compressing on the nerve and that is causing the nerve to constantly fire. There's still a lot of work being done is to find out the exact cause of why this starts. Sometimes is on investigation we do not find any particular reason then we call it idiopathic. To rule out any neurosurgical cause we generally have to do an MRI and go along to trace along the facial nerve to see if there's any surgical cause or like a vessel causing compression which might need a surgical intervention. Once that is ruled out then we can go for other treatments. So rarely muscle spasms may also be a part of Meek Syndrome. It can also be a part of Parkinson's disease it can also be a part of Bell's paly where there is irregular growth of nerve fibers causing the patient to have muscle contractions. When it's not required like he's trying to make another facial expression and his muscle is causing him to smile or his eyes causing him to close . We can also have tardive dyskinesias and we can also have Wilson's disease which can cause these kind of facial spasms. So these will also be need to ruled out when the patient presents to us. No hemifacial spasms or blepharospasms will not cause blindness because they do not affect the eye or the vision per se. It just causes disturbances in vision or dry eyes because of this irregular movements of the muscle. So facial muscle spasms once we've ruled out any organic cause we can consider giving botox injections. These injections will reduce the muscle twitching because it acts at the neuromuscular junctions and generally this treatment lasts for about 3 to 4 months depending on the body type of the patient. Rarely sometimes when the patient is not responding adequately to Botox or may require surgery like myomectomy where the muscles around the eyelid are operated for for reducing the number of muscle spasm. When we give Botox usually it lasts for about 3 to 4 months. Sometimes temporarily it can have some minor side effects where there is droopiness of the eyelids or there mild double vision. This generally resolves in about 2 weeks time. Otherwise botox does not have any significant side effects.
