Leaving Zote to die will get you the neglect achievement. No one seems to be sure what the exact trigger is, though.

I Killed Hornet, got the Mantis claw, opened Queen's Station, fell into Deepnest, and got the charm notch from the shroomal ogres. So I expect that one of those must be it.
