Primary Care and the New Medical QB
How are corporations rethinking wellness and prevention for their employees? Covid has been a wake-up call to remind people the best way to get through a pandemic is to be as healthy as possible. Those hardest hit by Covid were the ones with chronic conditions, many of which could have been avoided by healthy behaviors: eating well, staying physically active and emotionally resilient, avoiding addictive habits and getting enough sleep. More corporations are also waking up to the fact that they can be a big influence on these healthy behaviors, which ultimately lead to healthier outcomes and significantly lower medical bills. Moderators: Dr. Archana Dubey (CMO Consultant, HP; Chief Clinical Officer, AliveCor), Bambi Francisco Roizen (Founder and CEO, Vator); Speakers: David Utley (Founder and CEO, Carrot), Diana Han, MD (Chief Medical Officer, VP Global Health, Unilever), Antonio Tataranni, MD (Chief Medical Officer, PepsiCo), Omar Dawood (President, BetterUp Care)

Welcome to Invent Health (#inventhealth), a twilight salon-styled tech talk and mixer produced by Vator and UCSF Health Hub, where valuable lessons and insights are shared by investors, tech pioneers and adopters. Invent Health is designed to take a 360-degree view of one particular topic.
In this case, the topic is “Primary Care and the New Medical QB.” Primary care physicians are reporting burnout at twice the rate of other healthcare caregivers in primary care practices. The main reason is because they’re seeing 20 patients a day while now having the added burden of inputting their interactions into electronic health records. Adding insult to injury, they’re one of the lowest-paid doctors. This is reflected in the little amount spent for primary care. A 2019 study found that under 6% of healthcare spending went to primary care in the US compared to 14% for other OECD countries on average. The same report by The Robert Graham Center, which looks at policies around primary care, showed that increased investment in primary care reduces total healthcare spending. For example, in Rhode Island, a four-year effort to cap prices and expand primary care resulted in an increased primary care spend of $27 million but an overall reduction in costs of $115 million.
And yet, primary care physicians are the gateway to our health. Consider that 79% of antidepressants are prescribed by doctors, who more often than not prefer to spend more time with patients than just administer medications. As Dr. Archana Dubey says, the primary care system is “run like an assembly line and is an upside-down pyramid scheme of a model!”
So how do we change it? Many companies are trying to figure that out. K Health wants to provide primary care at $9 a month for 24/7 unlimited access to a clinician. In-person clinics, like One Medical and Forward, are reinventing the look and feel of a clinic so it’s more technologically-driven but also as inviting as a spa. The Well is another take on primary care focused largely on prevention. There’s also the telehealth-first approach with companies such as Doctor on Demand, Amwell and Teladoc, which is aggressively building upon its telehealth reach with its acquisition of Livongo, and its explosive BetterHelp division. Then you’ve got other emerging companies such as Ro and Hims/Hers, which like K Health, focuses on cash pay and specific conditions, like sexual health and smoking cessation.
It’s an evolving world with the primary care doctor potentially at the center as the new-age quarterback.
In 10 years, what will the doctor's office look like? Will it be integrated with your gym? What's the role of the medical doctor? How much will they matter if people self-diagnose already, and are now seeking more guidance about nutrition, mental health, lifestyle changes?

Government Health Initiatives: The CDC and the private sector were not prepared for the broad testing needed. What can policymakers and innovators do together to create a more rapid response protocol to a pandemic? COVID has exposed the gap in coverage with lower-income communities at a disadvantage. Are we leveraging the initiatives already in place to support underserved communities (healthcare, education, opportunity)?

How do we give each other space, but also engage better? What are some tips and best practices to adjust? Why is it important to find compromise and trust one another? Why is discovering new routines with our friends/family good for us?

Bambi Francisco Roizen kicks off event with welcome remarks:
This is an unprecedented time as the country shelters in place. Through this time we’ve had to make some pretty big sacrifices from homeschooling our kids, to being isolated from our friends and family, to losing work or a loved one.
Yet slowing down a bit might be good for us. Some of us are doing things we should be doing more of -- having sit-down dinners with family or meaningful conversations with friends, cleaning out our cluttered homes versus buying more stuff to fill it. It’s also good to be still, and reflective; to think outside ourselves and think outside the box; It’s also time to be grateful. So is this time actually having a positive effect? Or are the unknowns just too overwhelming?
Speakers include Alex Morgan (Investor, Khosla Ventures), Russell Glass (CEO, Ginger.io), Tim Blevins (SVP Behavioral Health, UnitedHealthCare Optum), Alon Matas (President, Teladoc’s BetterHelp); Moderators are Dr. Archana Dubey (Global Medical Director, HP Health Centers, HP), Bambi Francisco Roizen (Founder and CEO, Vator)
Would love your feedback on this event. Please post your thoughts to @vatortv @bambi100 #inventhealth
Russ Glass, CEO, Ginger: We saw a 25% increase in therapy and psychiatry sessions in the last month. The intensity is up about 11% as people's struggles are becoming more serious. The type of concerns people are having right now are around parenting, childcare, and financial.
Tim Blevins, SVP Behavioral Health, UnitedHealthcare Optum: We've doubled the network of clinicians to provide secure virtual care. Additionally, employer customers have asked us to waive member financial liability for routine outpatient behavioral care. Thirdly, we've seen a reduction of the number of people being admitted to hospitals. If there's any one group whom you don't want to be isolating, it's patients who are compromised psychiatrically. We're seeing engagement in relationship issues and couples therapy. I'm convinced this will bring families and people closer in general.
Alon Matas, President, Teladoc's BetterHelp: We typically see 2 million sessions a month, and we've seen a significant uptick from that. Our existing members are also utilizing the service much more. We're seeing a rise in stress and anxiety concerns. We're seeing differences across different states too, depending on how states are dealing with lock down.
Alex Morgan, Investor, Khosla Ventures: We have a number of companies working specifically on COVID-19, in terms of developing new diagnostics or therapeutic approaches.
Thanks to our sponsors: UCSF Health Hub, HP, Betterhelp, Avison Young, Advsr, Scrubbed, and Stratpoint.
But as we move out of this phase and start opening up the economy and move into a new normal, certainly not the old normal, the question is will we see another silent pandemic - this time a mental health pandemic and is our mental health system prepared?
Speakers: Brian Garrett (Co-founder & Managing Director, Crosscut Ventures), Dr. Nina Vasan (Chair of Innovation Lab, American Psychiatry Association), David Bond (Director of Behavioral Health, Blue Shield of California – Promise Health Plan), Alon Matas (President, Teladoc’s BetterHelp)

In this case, the topic is the “The future of clinics.” The CVS / Aetna deal is one example of how local retail chains are evolving to offer more care services. Upstarts like Forward, are taking a more futuristic tech-centric view of the new clinic; Heal is bringing the doctor to your home; Crossover Health is combining medical and wellness care in the corporate setting while the Well is redefining clinics to embrace wellness and lifestyle.
