There are four locations where you need to kill all of the enemies in an area that has a specific type of tombstone in it. Then you have to stand still in front of the tombstone where a ghost will appear to give you a piece of an item. After you get all four pieces you can give the put together item to the room full of tombstones located in the Icebound Mausoleum. Then you get a neat golden bile flask which fully heals you and fully fills your fervour as well.
