Update: You may not need all 8 penances in order to get all the rewards. My guess is that you need at least 6 in order to get the 6 rewards. As you put penances on at the start of the run the skull at the bottom right gets jewels and a crown. Once the skull gets the crown and all the jewels put on it and is no longer getting additional things on it if you add more penances then you should be good to get all the rewards. I don't know if this is 100% true, but from what I've read online this makes the most sense to me.
This was in my Incarnate Devotion Boss Fight video where I had all challenges (penances) active, but I thought I should probably just make it a separate video by itself as I'm sure people are wondering what the rewards are. Definitely want to mess with these rewards in the future. For now it's onto the DLC on True Torment Mode.
