We dive deep into how medications like Wegovy and Ozempic—while popular for weight loss—don't differentiate between fat and muscle loss, which can be crucial for long-term health. We'll also explore the growing evidence supporting ketogenic diets for their ability to promote fat loss while preserving muscle mass, making them a compelling option for improving body composition.
Watch this video to understand how optimizing your body composition, rather than just losing weight, can lead to sustainable health benefits.

- Introduction to Saturated Fat and Mental Health: Unpacking the myths and truths about saturated fats and their impact on health.
- The Flexibility of Ketogenic Diets: The keto diet involves achieving a state of ketosis. There are various dietary patterns that can shift the body's fuel source from glucose to ketones, and it is not necessary to consume saturated fat if one prefers not to.
- Saturated Fat Health Implications in Ketosis: Saturated fat in a high-carb diet, often found in processed foods like pizza and desserts, can lead to poor health outcomes. However, in a whole foods, low-carb ketogenic diet, saturated fat is consumed with nutrient-dense foods, and the body burns the fat as fuel, leading to improved metabolic health. There is no quality evidence proving that eating saturated fat on a keto diet would be harmful.
- Evidence-Based Analysis: A review of various studies and meta-analyses exploring the link between saturated fat intake and heart disease risk, with insights into the quality and limitations of the evidence.
- Research Insights: Most studies linking saturated fat to heart disease involve high-carb diets. In contrast, research on ketogenic diets shows significant metabolic improvements and reduced cardiac risk, highlighting the importance of dietary context.

According to Dr. Scher, any investigation into nutritional interventions for health conditions is a step in the right direction. Diets that steer away from the Standard American Diet (SAD) will enhance metabolic health by eliminating ultra-processed foods and sugar, likely offering both physical and mental benefits. However, ketogenic therapy isn't a conventional diet; it's an impactful metabolic treatment that modifies the brain and body's energy source. It's even possible to adopt a Mediterranean keto diet.
Key Points:
▪️Diet and Mental Health: Importance of diet in managing psychiatric symptoms.
▪️Ketogenic Diet: Recent studies and patient testimonials highlighting how ketogenic therapy can put serious mental illnesses, like major depressive disorder, bipolar disorder, and schizophrenia, into complete remission.
▪️Comparison and Recommendations: Advice on choosing between these diets and potential next steps for patients and clinicians looking to explore dietary interventions for mental illness.
▪️Future Directions: Ongoing research and the need for head-to-head trials to determine the most effective dietary approach for mental health.
Watch to Learn:
▪️How the Mediterranean and ketogenic diets impact mental health.
▪️The underlying mechanisms by which ketosis may be a powerful intervention for brain health.
▪️Practical tips for implementing dietary changes to support mental well-being.

Most notably, to fully benefit from a ketogenic diet, it's essential to restrict carbs enough to transition to a fat-based metabolism. Diets that are low-carb compared to the Standard American Diet (SAD) but not low enough to induce ketosis won't provide the unique benefits of a ketogenic diet, making the health outcomes incomparable.
Key Points:
- The study in question used an observational nutrition epidemiology approach, often considered to provide low-quality evidence.
- Low-carb was defined as less than 100 grams of carbs per day, which is not truly low-carb and definitely not ketogenic.
- The majority of participants provided only one 24-hour food diary over a decade, a poor representation of long-term eating habits.
- The study participants who ate low-carb were generally less healthy to begin with, skewing results due to the healthy user bias.
- There was no clinically significant increase in LDL cholesterol between the groups.
- The hazard ratio for cardiac events was 1.3, which is marginal compared to stronger associations like smoking and cancer.

Dr. Bret Scher clarifies in this video that single-arm pilot studies are not intended to offer conclusive evidence but rather to identify promising areas for future, more comprehensive research. Globally, gold-standard Randomized Controlled Trials (RCTs) are underway to further investigate ketogenic therapy for severe mental illnesses, aiming to establish a stronger evidence base and elucidate underlying mechanisms.
Apart from compelling patient testimonials supporting the efficacy of medical keto far beyond weight loss, there are scientific grounds to hypothesize its potential as a treatment for psychiatric disorders. The established success of ketogenic therapy in managing seizures, attributed to mechanisms such as ketones providing an alternate fuel source for the brain, reducing neuroinflammation, and optimizing mitochondrial function, underscores its neurological benefits.
The pilot study findings, coupled with compelling patient testimonials and insights from epilepsy treatment, underscore the importance of continued research and the potential benefits of ketogenic therapy for patients and clinicians wanting to manage symptoms.

Metabolic therapies, like a medical keto diet, can treat conditions like type 2 diabetes and obesity, as well as mental illnesses like major depression, bipolar disorder, and schizophrenia. The effectiveness of metabolic interventions likely results from repairing the metabolic dysfunction that underlies most disease states.

Jan Ellison Baszucki founded Baszucki Group and Metabolic Mind after helping her son, Matt Baszucki, implement metabolic therapies, including a medical keto diet, and witnessing him put his treatment-resistant bipolar 1 disorder into remission. She moderated the discussion with panelists, Lauren Kennedy, Wesley Braden, and Hannah Warren, who shared their experiences implementing metabolic interventions, including ketogenic therapy.
Lauren founded the prominent YouTube channel Living Well with Schizophrenia. Her ongoing Keto and Metabolic Health Project 2024 has documented her journey of putting schizoaffective disorder, bipolar type, into remission with metabolic treatments. Wesley uses metabolic therapies to treat OCD and bipolar 2 disorder; he describes them as life-changing. He is now leveraging his tech marketing background in the metabolic health field as senior marketing manager with Virta, a company that assists patients with low-carb diets to treat type 2 diabetes. Hannah has a background in nonprofit social entrepreneurship. She started as the Mental Health Communications and Advocacy Manager at Metabolic Mind in November 2023. She has been using metabolic treatments, including ketogenic therapy, as her sole form of treatment since the summer of 2021, and her bipolar 1 disorder is in complete remission.
Clinicians and researchers in attendance had the opportunity to ask questions of the panelists and to learn more about the game-changing impact metabolic therapies can have on patients' lives. Scientists, clinicians, and individuals with lived experience can work together to raise awareness of metabolic therapies and to change the way that psychiatric conditions are understood and treated as metabolic disorders of the brain.

In this video, Dr. Bret Scher shares an example of a misleading article from 2019 published by the American Heart Association that suggests a correlation between low-carb diets and Atrial Fibrillation (AFib), a heart condition that causes palpitations and can even lead to heart attack or stroke. For anyone interested in a keto diet, it is critical to note that there is a difference between a low-carb diet and a ketogenic diet. The therapeutic properties of a medical keto diet are due to the physiologic state of ketosis, not simply from eating a diet that is relatively lower-carb when compared to the Standard American Diet (SAD). Participants in the study ate far too many grams of carbs to be consistently in a state of ketosis, and the study had confounding factors that could easily skew results.

In this video, Dr. Bret Scher goes over some of the most common conditions that many people falsely believe would interfere with safe implementation of a keto diet, including: gallbladder removal, bariatric surgery, gout, high cholesterol and heart disease. He explains that following a keto diet with these conditions is, in fact, safe, if approached in the right way with clinical supervision.
