This patient received severe burns to the face, chest, and arms. We used a high speed dermabrader to reach a skin layer where bleeding will occur. The burn areas on her arms were so deep that we needed to use a high speed circular knife to achieve pinpoint bleeding. We placed xenografts over the wound areas to promote skin growth and help this young girl heal. Skin that is burned through the full thickness loses both elasticity and sensation. Unfortunately, if the skin will not stretch and swelling is severe, underlying muscle, nerves and blood vessels can get compressed and compromised and this can led to gangrene. The immediate treatment is called an escharotomy, where an incision is made through the skin. Because the nerve endings in the burn tissue have been destroyed. This can be done quickly at bedside without anesthesia. This simple procedure can be limb and even lifesaving.
