NTSB Board Meeting - 2022 Fern Hollow Bridge Collapse NTSBgov 2024-02-23 | The National Transportation Safety Board held a virtual board meeting Feb. 21 to determine the probable cause of the collapse of the Forbes Avenue Bridge over Fern Hollow (also referred to as the Fern Hollow Bridge) in Pittsburgh, Pennsylvania.On Jan. 28, 2022, the 447-foot-long bridge experienced a structural failure and fell approximately 100 feet into the park below. Six vehicles were on or near the bridge when it collapsed and four people sustained injuries. Investigators found corrosion, deterioration, and section loss on all four of the bridge’s legs due to the continual accumulation of water and debris. This finding led the NTSB in May to issue an early safety recommendation urging the Federal Highway Administration to develop a risk-based, data-driven process to identify, prioritize, and perform any incomplete follow-up actions on bridges with uncoated weathering steel components. During the board meeting, the NTSB’s board voted on the final findings, probable cause, and recommendations. The board also voted on all changes to the draft final report. NTSB Animation – Overview of the collapse of the Fern Hollow Bridge NTSBgov 2024-02-21 | On Friday, January 28, 2022, about 6:37 a.m. eastern standard time, the Fern Hollow Bridge, which carried Forbes Avenue over the north side of Frick Park in Pittsburgh, Allegheny County, Pennsylvania, experienced a structural failure. As a result, the 447-foot-long bridge fell about 100 feet into the park below. The collapse began when the transverse tie plate on the southwest bridge leg failed due to extensive corrosion and section loss. The corrosion and section loss resulted from clogged drains that caused water to run down bridge legs and accumulate along with debris at the bottom of the legs, which prevented the development of a protective rust layer or patina. Although repeated maintenance and repair recommendations were documented in many inspection reports, the City of Pittsburgh failed to act on them, leading to the deterioration of the fracture-critical transverse tie plate and the structural failure of the bridge. At the time of the collapse, a 2013 New Flyer articulated transit bus, operated by the Port Authority of Allegheny County, and four passenger vehicles were on the bridge. A fifth passenger vehicle drove off the east bridge abutment after the collapse began and came to rest on its roof on the ground below. As a result of the collapse, the bus driver sustained minor injuries and two bus occupants were uninjured. Of the six passenger vehicle occupants, two sustained serious injuries, one sustained a minor injury, two were uninjured, and the injury status of one was unknown.

NTSB Board Meeting - 2021 Anchor Strike Underwater Pipeline / Crude Oil Release in San Pedro Bay, CA NTSBgov 2023-12-07 | The National Transportation Safety Board held a virtual public board meeting Dec 5, 2023 to determine the probable cause of a crude oil leak from a crack that had developed in a subsea pipeline located in San Pedro Bay, 5 nautical miles off the coast of Huntington Beach, California on Oct 21, 2021. This is a multimodal investigation with the Office of Marine Safety (OMS) and the Office of Railroad, Pipeline and Hazardous Materials Investigations. OMS is the lead office.The pipeline was shut down and isolated after oil was spotted on the surface of the water. About 588 barrels of oil are estimated to have leaked from the pipeline. A post accident examination of the pipeline found a crack running longitudinally along the top of the pipeline, and about 4,000 feet of the pipeline was displaced up to 105 feet to the southeast from its original location. Additionally, near the location of the pipe breach, the seafloor was scarred from apparent dragging anchors.During the meeting, NTSB board members discussed safety issues related to this leak as well as the need for improvements to Vessel Traffic Service vessel monitoring systems offenders. NTSB Webinar - Communicating and Connecting Safety Messages to Native American Communities NTSBgov 2023-08-15 | With over 574 federally recognized tribes, Native American communities are extremely diverse in location and culture. With their own laws and jurisdiction, data collection can be uniquely challenging to obtain. Crash data on Native Americans is underreported, and if reported, the statistics would likely be even higher. Privacy concerns, staffing limitations, and lack of equipment and training are all contributors to this struggle. Yet as we look at the data, Native American communities are a key demographic in need of outreach and advocacy.The National Highway Traffic Safety Administration NHTSA estimates 42,795 people died because of crashes in 2022, compared with 42,939 in 2021. But all communities and areas of the country are not affected equally. According to an article published by the Bureau of Indian Affairs (BIA), those who identify as American Indian or Alaska native are killed and injured at a rate 2 or 3 times the national average (BIA 2022 Annual Report). Motor vehicle crashes are the leading cause of unintentional death for American Indians and Alaska Natives (AI/AN) ages 1–44, according to the Centers for Disease Control and Prevention Roadway to Safer Tribal Communities Toolkit, as quoted in Federal Highway Administration’s FHWA’s Tribal Government Safety Data. This webinar is part three in our “‘Communicating and Connecting Safety Messages with Underserved Communities”' series, which supports Executive Order 13985: Advancing Racial Equity and Support for Underserved Communities Through the Federal Government.
